摘要
探讨Ⅱ型糖尿病下肢动脉病变患者血流动力学及生化指标的变化情况。方法将我院住院的60例Ⅱ型糖尿病患者按照病程分为A组(病程≤10 a)与B组(病程>10 a)。分别测量两组患者下肢髂外、股、胭及足背动脉内径、收缩期峰值流速、平均血流速度、血流量、踝肱指数以及常规生化项目,并进行比较。结果两组股动脉内径、股收缩期峰值流速、股平均血流速度、胴收缩期峰值流速、胭平均血流速度、足背收缩期峰值流速等结果比较差异均有统计学意义(P<0.05),其余指标比较差异均无统计学意义(P>0.05)。两组踝肱指数检测结果比较差异无统计学意义(P>0.05),生化指标水平比较差异无统计学意义(P>0.05)。结论10 a以上Ⅱ型糖尿病病史是下肢动脉病变的危险因素,病程对糖尿病患者生化指标无显著影响。股收缩期峰值流速、股平均血流速度、胴收缩期峰值流速、胭平均血流速度指标对于临床评判Ⅱ型糖尿病患者下肢动脉病变严重程度具有一定的临床意义。
Objective To investigate the changes of hemodynamics and biochemical parameters in patients with type 2 diabetes mellitus with lower extremity arterial disease. Methods 60 cases of patients with type 2 diabetes were divided into group A (duration ≤10 a) and group B (duration > 10 a) according to the course of the disease. The internal diameters of external iliac, femoral,popliteal and dorsal pedal arteries, peak systolic velocity, mean blood flow, blood flow, ankle-brachial index and routine biochemical i-tems of the two groups were measured and compared. Results There were significant differences between the two groups in internal diameter of femoral artery, peak velocity of femoral systole, average velocity of femoral blood flow, peak velocity of popliteal systole, average velocity of popliteal blood flow and peak systolic velocity of dorsum pedis (P<0. 05), and there was no significant difference in other indicators (P>0. 05). There was no significant difference in the results of ankle brachial index between the two groups (P〉0. 05),and no significant difference in biochemical indexes (P〉0. 05). Conclusion The history of type 2 diabetes mellitus over 10 years is a risk factor for lower extremity arteriopathy, and the course of the disease has no significant effect on the biochemical indexes of diabetic patients. The indexes of peak femoral systolic velocity, average femoral blood flow velocity, peak popliteal systolic velocity and average popliteal blood flow velocity have certain clinical significance in judging the severity of lower extremity arterial lesions in type 2 diabetic patients.
