摘要
目的:评价达明饮治疗糖尿病视网膜病变(Diabetic retinopathy,DR)的疗效及安全性。方法:在中国期刊全文数据库、中国生物医学文献数据库、万方医学数据库、中文科技期刊数据库、Pubmed数据库中,检索2000-2017年应用达明饮为干预措施治疗糖尿病视网膜病变的随机对照研究文献,由两名研究人员独立地对这些研究的基本特征和方法学质量进行评价,采用RevMan5.3软件对治疗效应(临床综合疗效、视力、眼底、视野)进行Meta分析。结果:共纳入12篇文献,共591例患者(1144只眼)。Meta分析结果显示,试验组临床综合疗效显著优于对照组,差异有统计学意义[OR=3.35,95%CI(1.68,6.66),Z=3.44,(P=0.0006)]。试验组对视力的改善情况显著优于对照组,差异有统计学意义[SMD=0.42,95%CI(0.27,0.57),Z=5.38,(P<0.00001)]。试验组眼底有效率显著优于对照组,差异有统计学意义[OR=3.33,95%CI(2.13,5.19),Z=5.30,(P<0.00001)]。试验组视野平均光敏度提高显著优于对照组,差异有统计学意义[MD=1.75,95%CI(1.42,2.09),Z=10.22,(P<0.00001)]。试验组视野平均缺损提高显著优于对照组,差异有统计学意义[MD=-0.36,95%CI(-0.47,-0.26),Z=7.08,(P<0.00001)]。结论:达明饮对治疗DR有一定疗效,但还需更多以及更高质量的RCT进行分析才能对其疗效得出肯定性结论。
Objective:To evaluate the curative effect and safety on diabetic retinopathy(DR)by the treatment of Daming Yin. Methods:Two researchers retrieve the literature of randomized controlled studies using Daming Yin as an intervention for diabetic retinopathy from 2000 to 2017 and independently evaluated the basic characteristics and methodological quality of these studies which are searched in China journal full text database,China biomedical literature database,WanFang medical database,Chinese science and technology journal database,Pubmed database,using Meta-analysis to analyze the therapeutic effects(comprehensive clinical efficacy,visual acuity,eye fundus and visual field)by RevMan5.3 software. Results:There were 12 literatures included and 591 patients(1144 eyes)enrolled. The results of Metaanalysis showed that the comprehensive clinical efficacy [OR=3.35,95%CI(1.68,6.66),Z=3.44,(P=0.0006)],the visual acuity[SMD=0.42,95%CI(0.27,0.57),Z=5.38,(P<0.00001)],the effective rate of fundus[OR=3.33,95%CI(2.13,5.19),Z=5.30,(P<0.00001)],the mean visual field defect [MD=-0.36,95%CI(-0.47,-0.26),Z=7.08,(P<0.00001)] and the average visual acuity [MD=1.75,95%CI(1.42,2.09),Z=10.22,(P<0.00001)] of all the experimental group were significantly better than that of the control group,and the difference were statistically significant. Conclusion:Daming Yin may have certain curative effect on treating DR,but more high-quality RCT analysis are needed to reach a positive conclusion on its efficacy.
