摘要
目的:观察中药清热通腑汤在急性胆囊炎腹腔镜术后的应用效果。方法:选取腹腔镜手术治疗的急性胆囊炎患者88例,以随机数字表将患者分为两组,每组44例。对照组术后予以常规抗感染、补液等对症支持治疗,观察组在术后常规治疗的基础上应用清热通腑汤,每日1剂口服,用药1周后,比较两组患者术后胃肠功能的恢复情况,并于给药前后分别ELISA检测两组患者血清炎症因子TNF-α(肿瘤坏死因子)、IL-10(白细胞介素10)水平,记录两组患者并发症发生情况。结果:观察组患者术后肠鸣音恢复时间、肛门排气时间、排便时间均短于对照组(P <0. 05)。术后1 d两组患者的血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-10(6) IL-10)水平差异不显著(P> 0. 05),术后5 d两组的血清CRP、TNF-α、IL-10水平均有所7)降低(P <0. 05),观察组三者水平低于对照组(P <0. 05)。观察组并发症发生率低于对照8)组(P <0. 05)。结论:在急性胆囊炎腹腔镜术后应用清热通腑汤,对于促进患者的胃肠功能9)恢复,减轻炎症,减少并发症积极作用。
Objective:To observe the application effect of Qingre Tongfu Tang in postoperative laparoscopy patients with acute cholecysfitis.Methods:88Cases with postoperative laparoscopy of acute cholecystitis were divided into two groups according to random digit table,control group (44cases)were treated with Western medicine routine treatment that concluded anti -infection and fluid infusion,etc.,observation group (44cases) were given oral Qingre Tongfu Tang on the basis of control group,treatment lasted for 7days,then compared the recovery of patients'gastrointestinal function,determined serum TNF-α and IL-10 levels after operation 1 and 7days,recorded the incidence of complication.Results:The time of bowel sound ,exsufflation and defeca- tion of patents in observation group were shorter than those of control group (P <0.05).Serum TNF -α and IL -I0levels of the two groups had no significant differences in the first day after operation (P >0.05),they were decreased in the fifth day after operation,the levels of observation group were lower than those in control group (P <0.05).The incidence of complication in observation group was significantly than that of control group (P <0.05).Conclusion:Applying Qingre Tongfu Tang in the treatment of postoperative laparoscopy patients with acute cholecystitis,that can promote the recovery of gastrointestinal function,alleviate inflammation and reduce complication.
