摘要
目的 :了解地区性消化性溃疡流行病学特点。方法 :对 16年间经胃镜检查诊断为消化性溃疡的病例进行统计分析。结果 :胃镜检查 4 718例 ,检出消化性溃疡 (PU) 10 2 5例 ,胃溃疡 (GU) 2 76例 ,复合性溃疡 (CU) 30例 ,十二指肠溃疡 (DU) 715例 ,其它溃疡 4例 ;溃疡患者男女之比为 5∶1;平均年龄 4 3 8岁 ,胃溃疡、复合性溃疡 5 0～ 6 0岁居多 ,而DU以 5 0岁以下居多 ,各部位的溃疡随年龄增长检出率均逐渐升高 ,其中DU检出 30～ 4 0岁即达到高峰 ,而GU、CU高峰在 5 0～ 6 0岁 ,6 0岁以上患者DU和GU的构成比、检出率差异无显著性 (P >0 0 5 ) ;各季节检出率依次为冬 2 7 8%、春 2 1 4 %、夏 2 0 5 %、秋 18 9% ;长期趋势以 5年为一个时间单位检出率分别为 2 6 5 %、2 5 1%、17 0 % ,差异有非常显著性 (P <0 0 1)。结论 :(1)病变部位以十二指肠溃疡最多 ,胃溃疡次之 ,复合性溃疡较少 ;男性患者明显多于女性 ;(2 )GU、CU、DU检出率随年龄增长而增高 ,DU发病高峰年龄较GU、CU早 ,青壮年以DU多见 ,老年人DU、GU同样多见 ;(3)PU发病有一定的季节性 ,但各季节均有较高的检出率 。
Objective:To find out the regional epidemiological feature of peptic ulcer.Methods:The cases of peptic ulcer diagnosed by endoscopy in recent 16 years were analysed statistically.Result: of 4718 endoscopic cases,1025 cases of peptic ulcer were found.Among them,276 cases were gastric ulcer,30 cases were composite ulcer,715 cases were duodenal ulcer,and 4 cases were other peptic ulcer.The ratio of male to female patients was 5∶1.The average age was 43 8 years.Most of the gastric and composite ulcer patients were at the age of 50～60,while most of duodenal ulcer patients were under the age of 50.The detecting rates of gastric,composite and duodenal ulcer was inceasing with age.Among them,the peak rate of duodenal ulcer was at the age of 30～40,while peak rate of gastric and composite ulcer was at the age of 50～60,At the age of over 60,constituent ratio and detecting rate of gastric and duodenal ulcer showed no significant difference(P>0 05).Detective rates of peptic ulcer in winter,spring,summer and autumn were 27.8%,21 4%,20 5% and 18 9% respectively.When 16 years were devided to three periods (average 5 years for one period),detective rates of per 5 years were 26 5%,25 1% and 17 0% respectively.The positive rates between them showed significant difference (P<0 01).Conclusion:(1) As for the position of peptic ulcer,duodenal ulcer is far more than gastric ulcer,and composite ulcer is the least,The male patiens are far more than female.(2)Gastric,composite or duodenal ulcer are increasing with age.The peak of ill age in duodenal ulcer is earlier than that in gastric and composite ucler.Most of duodenal ulcer cases are youth,and in the old people,the cases of duodenal ulcer are really as more as gastric.(3)Peptic ulcer attacked seasonally.but detective rates in each season are higher.In the long time,detective rates of peptic ulcer are getting down.
