摘要
目的探讨舌下含服米索前列醇预防剖宫产产后出血的效果。 方法将妊娠晚期单胎选择性剖宫产者 2 0 0例随机分为两组 ,研究组 110例 ,在第二产程末胎头娩出后舌下含服米索 4 0 0 μg ;对照组 90例 ,在胎儿娩出后静脉注射催产素 2 0IU。观察产后 2h出血量和用米索前后血压及用药后出现的副作用。 结果产后 2h内的出血量 ,研究组与对照组分别为 174 .4 3ml和 2 19.73ml,二组比较有显著性差异 (P <0 .0 1)。用米索前后血压比较 ,二组无显著性差异 (P>0 .0 5 )。 结论舌下含服前列醇促进子宫收缩作用强于催产素 ,能减少产后出血 ,也适用于某些与血管变化有关的高危孕产妇 。
Objective To observe the effect of subligual misoprostol tablets on the prevention of postpartum bleeding of cesarean section. Methods Two hundred patients in late single pregnancy selective cesarean section were divided into two groups: 110 patients in the studied group and 90 patients in the control group. In the studied group, 400μg misoprostol was given sublingually after deliverly of the fetal head at the end of the second duration of labor. In the control group 20 IU oxytocin was given intravenously after delivery of the fetus. The amount of bleeding within 2h after delivery, blood pressure change before/after giving misoprostol and side effects were observed. Results The amount of postpartum bleeding within 2h was 174.43ml and 219.73ml in the studied group and control group, respectively. The difference in the amount of bleeding between two groups was obvious(P<0.01). The blood pressure change before/after misoprostol intake was not obvious (P>0.05). Conclusion Misoprostol is more effective than giving oxytocin on the effect of uterine contraction. It can decrease hemorrhage. Its administration is suitable to prevent postpartum hemorrhage for high risk pregnancy with some vascular changes.
