摘要
目的 :观察腰椎间盘脱出患者神经根损害的神经电生理表现。方法 :对 4 5 1例临床确诊的腰椎间盘脱出患者的针电极肌电图 (EMG)、神经传导速度 (NCV)、体感诱发电位 (SEP)进行分析。结果 :EMG异常 2 98例 (6 6 1% ) ,均进行手术治疗 ;胫神经H反射异常 16 5例 (36 6 % ) ;NCV异常 16例(3 5 % ) ,提示同时伴有周围神经病 ;SEP异常 372例 (82 6 % ) ,以P4 0潜伏期延长为特点 ,而 3例合并脊髓压迫的患者主要为波幅降低或未引出。结论 :EMG、NCV、SEP三项联合检测 ,不仅可以提高腰椎间盘脱出患者神经根损害电生理检测异常率 ,还可以对神经根损害程度、范围以及对周围神经。
Objective:To observe the electroneurophysiological characteristics of nerve root damages in patients with lumber intervertebral disc extrusion.Methods:The needle pole electromyography (EMG),nerve conduction velocity(NCV),and somatosensory evoked potential (SEP) were analyzed in 451 patients with lumbar intervertebral disc extrusion diagnosed clinically.Results:The EMG abnormality was found in 298 patients,the abnormality rate was 66.1%.The abnormality of H reflect was in 165 patients with a abnormality rate of 36.6%.The abnormality of NCV was noted in 16 ones (3.5%),showing that the 16 patients were accompanied by peripheral neuropathy.The abnormality of SEP was found in 372 ones,characterized by the prolongation of P40 latency,the abnormality rate was 82.6%,but in 3 patients with spinal cord compression,the characteristic of SEP was the decreace of wave amplitude or disappearance of the waveform.Conclusions:Combination detection of EMG,NCV and SEP might increase the abnormality rate of electroneurophysiological detection in nerve root damages in patients suffered from lumbar intervertebral disc extrusion,and provide reliable evidence for the diagnosis of degrees and range of damages to the nerve root,as well as the peripheral nerve and spinal cord.
