摘要
目的 探讨儿童人工晶状体植入的最佳时机、屈光度选择及并发症的防治。方法 5 7例 6 7眼先天性和外伤性白内障 ,年龄 2岁至 12岁 ,行白内障摘出并后房型人工晶状体植入术 ,术后随访 2～ 14月。结果 除 10例患儿不合作 ,其余患儿术后视力均较术前有所提高 ,≥ 0 5为 2 8眼 ,占 5 3%。术后并发症主要为后囊浑浊 ,2 3眼行Ⅱ期后囊切开或YAG激光治疗。其他并发症包括葡萄膜炎、瞳孔移位、人工晶状体瞳孔夹持等。结论 人工晶状体植入术是目前儿童白内障较为理想的复明手段 ,但应掌握手术时机 。
Objective Reporting 67 eyes which were implanted posterior chamber artificial lenses.Discussing opportunity of implantation,choice of diopter,prevention and treatment of complications.Methods Extracapsular cataract extraction(ECCE) and posterior chamber IOL operations were performed in 57 cases(67 eyes) of congenital and traumatic cataractaged from 2 years to 2 years old.The follow-up ranged from 2 months to 14 months.Results The visual acuity was improved in most cases after surgery except 10 in which the follow-up was lost.53%of the eyes achieved a visual of 0.5 or better.The main complication was posterior capsule opacification,accounting for 70%. Secondary capsulotomy or YAG laser treatments were performed in 23 eyes.The other complications included uveitis,dislocation of IOL.Conclusion IOL implantation is a better method for the recovery of visual acuity in pediatric cataract at present,but the opportunity of implantation should be considerd,the postoperative complications should be well managed.
