摘要
目的：探讨内窥镜鼻窦手术（ＥＳＳ）中保留或切除中鼻甲对筛窦术腔愈合以及对鼻中隔粘膜的影响。方法：① 选取３９例慢性鼻窦炎病人（Ⅰ型２期６例，Ⅰ型３期１例，Ⅱ型２期１７例，Ⅱ型３期９例，Ⅲ型６例，不含Ⅰ型１期、Ⅱ型１期）。采取同一患者一侧切除中鼻甲，而另一侧保留中鼻甲的方法；② 应用鼻内窥镜观察中鼻甲保留侧和切除侧筛窦术腔愈合时间、筛窦术腔闭塞和鼻中隔鼻甲的发生率。结果：① 中鼻甲保留侧和切除侧，两侧筛窦术腔愈合时间分别平均为２．９０个月和４．４７个月；② 两侧鼻中隔鼻甲出现率分别为１０．２％和３３．３％；③ 两侧筛窦术腔闭塞率分别为１５．３％和４６．２％。结论：内窥镜鼻窦手术中应尽可能保留中鼻甲，对筛窦术腔的愈合和减少筛窦术腔闭塞有积极作用。 ］
Objective:To investigate the clinical significance of reservation or remove of middle turbinate on the ednoscopic sinus surgery in 39 patients with chronic sinusitis.Methods:There were 28 males and 11 females,ranging in age from 11 to 60 years old.The clinical stage of patients were type Ⅰ stage 2 in 6,type Ⅰ stage 3 in 1,type Ⅱ stage 2 in 17,type Ⅱ stage 3 in 9,type Ⅲ in 6 .The management of middle turbinate was reservation in right site(group A),remove in left site(group B)in same patient during endoscopic sur gery.The healing times of ethmoid sinus cavity postoperation,the proportion of the septal turbinate formation and the closure rate of ethmoid sinus were observedundergo endoscopic technique.Results:The average healing times of group A and gr oup B was 81.60 days and 122.10 days respectively.The proportion of septal turbina te formation was 10.2% and 23.3% respectively.The proportion of the closure of ethmoid sinus was 15.3% and 46.2% respectively(P<0.01).Conclusion:Result sh owed that the reservation of middle turbinate should be beneficial to the healing of th e ethmoid sinus cavity and decrease the incidence of the closure of ethmoid sinus cavity.
