新型迭形接骨板的实验研究与临床应用 被引量:1

摘要 目的 改进骨折接骨板内固定技术 ,克服目前临床上使用的接骨板的一些缺陷 ,观察新型迭形接骨板临床效果。方法 根据汽车钢板弹簧结构和原理 ,依据等强度梁力学原理 ,研制一种新型迭形接骨板 ,进行有关力学实验研究和临床治疗四肢长管骨骨折。结果 力学研究与临床应用结果显示 ,该接骨板较好地克服了普通接骨板应力集中和高刚度等问题。临床上基本不发生螺钉和接骨板断裂现象 ,手术后大多不需外固定 ,并发症少 ,骨折愈合良好 ,临床效果满意。结论 该接骨板具有如下优点 :①结构新颖 ,由多层薄片钢板重迭而成 ;②应力分布非常均匀 (接近 1∶1) ;③应力保护作用小 ;④高强度、低刚度 ;⑤可组合性 ;⑥使用方便 ,容易推广。该接骨板较好地改进了四肢长管骨 (尤其是下肢 )骨折接骨板内固定技术 。 Objective To improve internal fixation technique for the treatment of fractures,to solve some problems of the plates used recently for the treatment of the long bone fractures and to study clinical results of a new overlapping bone plate Methods According to the structure and principle of car plate spring and the mechanical principle of equal strength beam,a new overlapping bone plate was designed and tested in experimental study for its mechanical property;121 patients with long bone fractures were treated with these plates.Results Mechanical and clinical study showed that the new plate well overcame some problems of traditional bone plates such as stress concentration and high stiffness.Clinically,broken screws and bone plates were not observed.The clinical results were good with little complications,no external fixation was used and bone union was satisfactory.Conclusion Compared with traditional plates,the new plate had the following advantages:the structure of this new type with duplication of many layers of thin slice steel giving very even stress distribution (near to 1∶1);decreased stress shielding effect;high strength and low stiffness;easy for use in combination;convenience in practical application.This plate is very suitable for the treatment of long bone fractures of low limbs.
