摘要
目的 探讨慢性及晚期血吸虫病患者门脉血流动力学变化及其与肝纤维化分级的关系。 方法 利用多普勒超声技术检测了 10 0例慢性及晚期血吸虫病患者及 2 4例正常人门脉血流动力学状态 ,将慢性及晚期血吸虫病患者按超声肝纤维化分级并分析血流动力学变化与其的关系。 结果 与正常对照组比较 ,门脉血流动力学检测血吸虫病组患者门静脉 (PV)内径明显增宽 ,血流速明显减慢 ,血流量明显增加 ,差异有显著性 (P<0 .0 0 1) ;脾静脉 (SV)和肠系膜上静脉(smv)内径 (D)、血流速 (V)、血流量 (Q)也有类似改变。超声肝纤维化分级为 级、 级、 级血吸虫病患者的 PV内径 ,按 级、 级、 级顺序显示门静脉宽度逐渐增宽 , 级的 Dpv较 、 级均有显著性增宽 (P<0 .0 5 ) , 、 级差异无显著性 (P>0 .0 5 )。 级较 级也有增宽趋势 ,按 级、 级、 级顺序显示门静脉血流速度逐渐降低 ,且各级间比较差异有显著性 (P<0 .0 1)。 级、 级、 级的 Qpv按 级、 级、 级顺序显示血流量逐渐增大 , 级较 、 级均有显著性增大 (P<0 .0 5 ) , 、 级差异无显著性 (P>0 .0 5 )。
Objective To analyse the realtionship between the changes of the portal system hemodynamics and the liver fibrosis grade of patients with chronic and advanced schistosomiasis. Methods The color Doppler ultrasound function was used to detect the portal system hemodynamics of 100 patients with chronic and advanced schistosomiasis and 24 normal persons, and 100 patients were divided by the liver fibrosis grade. Results There were significant changes on the portal system hemodynamics of patients with chronic and advanced schistosomiasis compared with normal group. The diameter of the portal vein(Dpv) ,the splenie vein(Dsv) and the superior mesenteric vein(Dsmv); the velocity flow of the portal vein(Vpv), the splenie vein(Vsv) and the superior mesenteric vein(Vsmv); the quantum of the portal vein(Qpv), the splenic vein(Qsv) and the superior mesenteric vein(Qsmv) were significantly increased( P <0.001) in schistosomiasis group, respectively. The Dpv of patients with fibrosis grade Ⅲ was wider than the those of fibrosis grade Ⅰand Ⅱ ( P <0.05), the Spv of patients with fibrosis grade Ⅲ was more decreased than the those of fibrosis grade Ⅰand Ⅱ ( P <0.01), moreover, the Qpv of patients with fibrosis grade Ⅲ was greater than the those of fibrosis grade Ⅰand Ⅱ ( P < 0.05 ). Conclusion Detection of the portal system hemodynamics of patients with chronic and advanced schistosomiasis can evaluate the degree of liver fibrosis and the portal hypertension. [
