摘要
目的 探讨穿刺针与肾之间切线力产生的原因、大小、影响因素及与肾穿刺成功率的关系。方法 6 1例疑肾脏疾病患者在B超引导下行肾穿刺活检 ,分别采用平静吸气后屏气 ,穿刺针尽量与肾表面垂直的改进法 ,以及深吸气末屏气与皮肤垂直进针的传统法。同时对 10例健康者行MRI检查 ,了解与切线力相关的不同呼吸状态下肾脏改变的解剖学基础。结果 穿刺针与肾之间的角度是产生切线力的原因。切线力的大小一是与穿刺针和肾表面间角度有关 ,二是与肾脏长轴和人体长轴间角度有关。改进法穿刺针尽可能与肾表面垂直 ,二者之间角度增大 ,同时平静吸气后屏气 ,肾脏长轴与人体长轴角度减小 ,使切线力减小 ,因而在肾穿刺成功率、平均穿刺针数等方面均优于传统法。结论 穿刺针与肾之间切线力的存在是影响肾穿刺成功率的重要因素 ,减小切线力 。
Objective To study the cause, strength and influential factors of the tangential stress between needle and kidney, and the relation with success rate of renopuncture.Methods Sixty-one patients received traditional and modified renopuncture techniques respectively. The traditional technique was to enter the puncture needle vertically to skin at breathholding after deep inspiration. The modified technique was to enter the puncture needle vertically to kidney at breathholding after calm inspiration. Meanwhile 10 volunteers received MRI examination to know the relation between the anatomic changing of kidney at two different respiratory states and tangential stress.Results Angle between puncture needle and kidney played an important role in tangential stress. The strength of tangential stress showed relation with the angle between kidney and puncture needle, and the angle between renal long axis and the long axis of body. In modified group puncture needle vertically to kidney increased angle between needle and kidney, which could make the angle smaller between renal long axis and body long axis at breathholding when calm inspiration and reduce tangential stress. The modified technique was superior to the traditional technique in the respect of the success rate and the average puncture needles. Conclusions The tangential stress between needle and kidney is the important factor affecting the success rate of renopuncture. To reduce tangential strees could increase the success rate of renopuncture.
