摘要
目的:通过MRI对小脑扁桃体位置的测量,为枕大孔区病变的影像学诊断提供诊断标准。材料和方法:在525例1~80岁正常人颅矢状MRI上,按不同性别和年龄进行分组,测量小脑扁桃体最下缘至枕大孔前后唇下缘连线基线的垂直距离,并进行统计学分析。结果:男性小脑扁桃体下缘至基线的平均距离为06±18mm-19±18~29±31mm,女性的平均距离为08±19mm-20±16~24±21mm,两组间比较无显著性差异P>005,相邻年龄组间比较无明显差异P>005,相间年龄组间40岁以前比较有显著性差异P<005,小脑扁桃体的位置有随年龄增长而逐渐上升的趋势,30岁以后趋于恒定。结论:小脑扁桃体的位置有随年龄变化而变化的特点,在对小脑扁桃体下疝畸形的诊断中,单纯距离测量是不恰当的,年龄也是值得考虑的因素。
Objective:To provide the criterion for the normal position of cerebellar tonsil relative to foramen magnum through MRI measurement.Materials and Methods:Five hundred and twenty five individuals aged one year to 80 years were grouped according to sex and age.The position of the cerebellar tonsils relative to the foramen magnum was measured on parasagittal MR images in all patients.The data were analyzed statistically.Results:The mean distance of the cerebellar tonsils from the foramen magnum in males was 0.6±1.8mm,ranged from -3.9 to 5.6mm,in females was 0.8±1.9mm,ranged from -4.3 to 7.1mm.There was no significant difference between males and females(P>0.05).There was slso no significant difference between two adjacent groups(P>0.05).However,there was statistically significant difference between separating groups before age 40(P<0.05).Tonsils ascended with increasing age and became steady after aged 30.Conclusion:There exists a characteristic fact that is the position of the cerebellar tonsils changes with age.In the diagnosis of Chiari type I malformation,it is inappropriate to consider only the distance of cerebellar tonsil relative to foramen magnum unless the factor of age is also considered.
