纯红细胞再生障碍和再生障碍性贫血的免疫抑制治疗 被引量:2

Immunosuppressive therapy for pure red cell aplasia and aplastic anemia

摘要 目的观察免疫抑制剂对纯红细胞再生障碍(PRCA)、重型再生障碍性贫血(SAA)与非重型再生障碍性贫血(NSAA)的治疗作用。方法采用小剂量环孢素A(3~5mg/kg.d)治疗7例PRCA、4例SAA及4例NSAA,动态监测环孢素A的血药浓度。从外周血象的变化,来判断环孢素A的临床疗效。结果环孢素A治疗3个月后,除1例SAA外,其余14例患者的血红蛋白水平均有较大程度的提高。随着治疗时间的延长,7例SAA与NSAA的白细胞数、中性粒细胞绝对数及血小板数也呈逐渐上升趋势。结论环孢素A治疗PRCA、SAA及部分NSAA的近期效果良好。 Aim To observe the therapeutic effect of immunosuppressive agent on patients with pure red cell aplasia(PRCA),severe aplastic anemia (SAA) and nonsevere aplastic anemia(NSAA). Methods Low-dose cyclosporine A(3~5mg/kg.d) was administered orally to treat 7 cases of PRCA ,4 cases of SAA and 4 cases of NSAA,and the whole -blood concentration of cyclosporine A was monitored dynamically throughout the treatment. The efficacy of cyclosporine A was judged by the changes of peripherial blood cell count. Results After cyclosporine A treatment for three months,the hemoglobin levels raised considerably in all patients except 1 case of SAA. With the treatment prolonged,moreover, white blood count, absolute neutrophil count and blood platelet count all increased gradually in 7 case of SAA and NSAA. Conclusion Cyclosporine A is effective for the treatment to PRCA, SAA and NSAA with satisfying short-term efficacy.
