摘要
目的 探讨逆行灌注心脏不停跳法心肌保护作用的效果。方法 用离体兔心工作模型 ,经冠状静脉窦逆行灌注 3 2℃稀释血液或血钾停搏液 12 0min。将 2 0只长耳大白兔随机分成 2组 ,每组 10只。A组 (心脏不停跳组 ) ,B组 (温血心停跳连续灌注液组 )。观察左室收缩压 (LVSP)和左室舒张末压 (LVEDP)、磷酸肌酸激酶 (CK )、左右心室三磷酸腺苷 (ATP)、丙二醛 (MDA)等。结果 A组的LVSP、LVEDP恢复率分别为 91.69%、111.3 1% ;ATP[(11.45± 0 .91)nmol/g干重 ]高于B组 (P <0 .0 5 ) ;MDA(2 .3 4± 0 .5 7) μmol/g和CK漏出率 (3 3 .69± 3 .0 9)IU /5mim .g干重低于B组 (P <0 .0 5 ) ;左右室心肌各项指标差异无显著性 (P >0 .0 5 )。结论 逆行灌注心脏不停跳法对左右心室均有良好保护作用 ,是一种安全有效的心肌保护方法。
Objectives To evaluate the myocardial protective effects of retrograde coronary sinus perfusion in beating heart.Methods Isolated rabbit working heart models were used in this study.Fourty 3 to 4 months rabbits were rondomly divided into 2 groups: group A (beating heart with continuous 32?℃ warm blood perfusion,n=10),group B (continuous 32?℃ warm blood cardioplegia arrest,n=10 ).Left ventricular systolic pressure (LVSP),left ventricular end diastolic pressure ( LVEDP) were measured.Creatine kinase (CK) leakage from blood samples in coronary sinus,ATP and malondialdehyde(MDA)in left and right ventricles were assessed 120?min after continuous perfusion through retrograde coronary sinus.Results Recovery rate of LVSP,LVEDP in the group A were 91.69% and 111.31%.ATP was (11.45±0.91)?nmol/gdw in the groups,higher than in the group B (9.98±0.85)?nmol/gdw,P<0.05).The levels of MDA(2.34±0.57?nmol/mg)and CK leakage [33.69±3.09?IU/(5?min.gdw)] were lower singnificantly in the group A than in the group B (P<0.05).There was no significant difference in the above parameters between the left and right ventricles in the group A (P> 0.05). Conclusion Retrograde coronary sinus perfusion in beating heart has good protective effects on both left and right ventricles and is a safe and effective method for myocardia.
