摘要
目的 探讨老年心肌梗死临床病理的性别差异。方法 对 84例尸检证实的老年人心肌梗死分为男女两组 (男 6 5例 ,女 19例 )进行临床病理对照分析。结果 老年女性心肌梗死平均年龄 (81 3岁 )大于男性 (72 7岁 ) ;女性心原性休克 7例 (36 8% )多于男性的 6例 (9 2 % ,P <0 0 1)。女性冠状动脉明显狭窄的平均支数为 1 7支 ,少于男性的 2 3支 ;女性三支以上冠状动脉明显狭窄 3例(15 8% ) ,低于男性的 2 8例 (43 1% ,P <0 0 5 ) ;女性急性初发心肌梗死及心脏破裂分别为 9例(47 5 % )及 6例 (31 6 % )均多于男性的 13例 (2 0 % )及 8例 (12 3% ) ,P <0 0 5。结论 提示老年女性心肌梗死的特点可能为患病年龄大、冠状动脉明显受累分支较少 ,急性初发心肌梗死。
Objective To investigate the gender differences in clinicopathologic changes of myocardial infarction in elderly patients Methods Eighty four elderly patients with myocardial infarction (19 females, 65 males) confirmed by necropsy from January 1980 through February 1998 were analyzed to compare the clinicopathologic states of female with male patients Results The mean age of female patients was higher than male (81 3 vs 72 7 years) More female patients were complicated with cardiogenic shock [7 of 19(36 8%) vs 6 of 65(9 2%), P <0 01] The mean number of major epicardial coronary arteries significantly narrowed (>75%) by atherosclerotic plaque was less in female patients than in male (1 7 vs 2 3) Three or more major epicardial coronary arteries were significantly narrowed in 31 patients [3 of 19 females (15 8%) vs 28 of 65 males (43 1%), P <0 05] Nine females [(47 5%) vs 13 males (20%), P <0 05] were of first attack, and cardiac rupture also occurred more frequently in females [6 of 19 (31 6%) vs 8 of 65 (12 3%), P <0 05] Conclusion The results suggest that the age of onset is older and the number of diseased artery is less in female than in male, but there is more frequent cardiogenic shock, first acute myocardial infarction and cardiac rupture in elderly female patients of myocardial infarction
