摘要
目的 探讨畸变产物耳声发射测试对噪声性聋的诊断价值。方法 对 115名军人 (2 0 4耳 )进行DPOAE幅值测试 ,采用两个等强度L1=L2 =70dBSPL的纯音信号f1、f2 作为刺激声 ,f2 /f1=1.2。测试 1、2、4kHz和 6kHzDPOAE幅值 ,根据DPOAE幅值是否大于或小于标准值 ,判断听力正常与否。并同纯音听阈进行对照。结果 1、2、4和 6kHz各频率的敏感度分别是 70 %、6 7%、85 %和 73% ;特异度分别是 71%、6 6 %、84%和 75 %。结论 DPOAE幅值测试用于诊断噪声性聋 ,缺乏足够的敏感性和特异性 ,单一的DPOAE幅值测试 ,不能诊断噪声性聋。
Objective To study the clinical efficacy of screening for noise induced hearing loss(NIHL) with distortion product otoacoustic emissions(DPOAE).Methods DPOAEs were recorded in 204 ears of 115 military personnel with the stimulus levels of the two tones equal to 70 dB SPL and the ratio between the two frequencies 1.2.In the DPOAE test, the cochlear function of one ear at a test frequency was predicted to be normal or abnormal depending upon whether the DPOAE level with the geometric mean of the two stimulus frequencies at test frequency was greater or less than a criterion. The DPOAE test outcomes were evaluated against the pure tone threshold as the standard. Results We found the sensitivity and specificity of the test to be 70%, 67%, 85%,73% and 71%, 66%, 84%, 75%, at 1,2,4,and 6 kHz respectively. Conclusion DPOAEs are not sufficiently sensitive to serve as a single test to identify NIHL.
