摘要
目的尝试一种分项目的纵向评定髋关节功能的方法———分项百分制髋评分,力求形象地反映患者术后髋关节的功能变化,便于临床随访及康复治疗。方法该评分是在Harris髋评分的基础上加以改良。改良内容包括:首先,将结果表达方式从总分表示改为以疼痛、功能和关节活动三个项目得分表示,每项为100分。每项实际得分非常直观地反映了患侧髋关节与正常髋关节在此方面的差距,有助于指导患者康复;其次,敏感性和重复性方面放弃影响因素多、结果不易肯定和术后康复指导不易纠正的评定内容,或减少它们的得分权重。在6年的临床应用中,我们评定了554例次髋关节。在临床评定时分别采用分项百分制髋评分和Harris髋评分,然后对评定结果进行Pearson相关系数统计学分析。结果分项百分制髋评分的平均分与Harris评分的总分之间有很好的相关性,说明两者有可比性。分项百分制髋评分较Harris评分略为苛刻。结论分项百分制髋评分比Harris评分更加形象地反映了术后患者髋关节的功能恢复情况,便于临床随访。
Objective To develop a subentry hip score system subentry percentage hip score in order to assess the hip function in a longitudinal direction, striving to visualize the imaging changes of hip status after hip arthroplasty for the conveniency of follow up and physical therapy. Methods The new hip score system was modified on the basis of Harris hip score method. The contents of modification included: A. The outcome assessment was expressed in the form of three variables instead of the average total score. The three variables involved of pain, functional activity and range of movement of the evaluated hip joint. Each variable contained 100 scores. Therefore, the variables could be used to judge the difference between evaluated hip and normal hip in the individual aspects and serve as a guide for physical therapy. B. The variables of sensitivity and repeatability, that were affected by multiplefactors, hard to confirm, or to correct during physical therapy were abandoned or their weight of scores were reduced. The status of a series of 554 hips were evaluated by using subentry percentage hip score and Harris hip score respectively during 6 years clinical trial. The data were analyzed by Pearson s correlation coefficient. Results The results showed good correlation and comparable between two hip scores (R >0.9). The subentry percentage hip score was a little more strict than Harris hip score.Conclusion The subentry percentage hip score is more visualized than Harris score to reflect the hip status postoperatively and easy to access during follow up studies.
