摘要
目的 :建立以单纯扩髓及髓腔内固定导致肺骨髓脂肪栓塞的大鼠模型。方法 :将 2 0只大鼠随机分为实验组和对照组 ,各组 10只。实验组除行双侧完整股骨转子窝处闭合扩髓及髓腔内固定外 ,均同对照组。两组术后均观察 110min ,处死大鼠并检查肺脏组织病理改变。按栓塞的肺小动脉及毛细血管数分为Ⅰ~Ⅳ级。结果 :实验组行单纯扩髓及髓腔内固定后 ,全部大鼠的肺小动脉及毛细血管可见扩张 ,管腔内充满骨髓干细胞及脂肪球 ,呈现骨髓脂肪栓塞改变 ;栓塞的等级为Ⅰ级 1只 ,Ⅱ级 3只 ,Ⅲ级 4只 ,Ⅳ级 2只。对照组未见肺骨髓脂肪栓塞。结论 :单纯扩髓及髓腔内固定大鼠骨髓脂肪栓塞模型简便易行 ,经济实用 。
Objective: To establish a rat model of pulmonary fat embolism with simple reaming and nailing. Methods: Twenty adult rats were randomly assigned to experimental (10 rats) and control groups (10 rats). After the anesthization with Nembutal through intraperitoneal and transpenil vein injection, canulation of left carotid artery and right jugular vein were undertaken. The canula of left carotid artery was connected with blood pressure monitor. The canula of right jugular vein was used for fluid transfusion to compensate the body liquid loss and maintaince of continuous anesthization with Nembutal during the whole process. After stabilization for 30 minutes, the animal in the experimental group underwent bilateral intact femoral reaming with 1.6mm drill and nailing with 1.2mm rod. All the animals were sacrificed 110 minutes postoperatively, and the lungs were examined histologically for the amount of embolized bone marrow which was graded Ⅰ to Ⅳ. The results were compared with those of equivalent specimen from 10 control rats that had undergone the same procedure except the femoral reaming and nailing. Results: Bone marrow fat embolism showed as hematopoietic cells and fat cells in pulmonary arteriole and capillaries after reaming and nailing in all rats of the experimental group. The results were: Ⅰgrade, one rat; Ⅱ grade, three rats; Ⅲ grade, four rats; and Ⅳ grade, two rats respectively. The control group had no bone marrow embolization. Conclusion: The rat model of pulmonary fat embolism induced by reaming and nailing was simple, reproductive and could further facilitate the clinical research of fat embolism.
