摘要
目的 分析子宫肉瘤误诊的常见原因。方法 对 1980～ 2 0 0 1年本院收治的 36例子宫肉瘤的诊断治疗进行分析。结果 1研究组 ( 组 ) 2 1例 (5 8.3%)为初次手术时误诊 ,本组患者有 14例再次手术 ,5例 3次手术 ,近期复发率 70 %。初诊确诊为子宫肉瘤者为对照组 ( 组 ) 15例 (4 1.7%)。2 组患者中位年龄较 组轻 ,误诊为子宫平滑肌瘤而行局部切除 ,增加复发率和血行播散的机会。 3 组再次手术的术中出血量及输血量较 组显著增加。结论 初次确诊并选择根治性手术方式是改善子宫肉瘤预后的关键。
Objective To analyze the causes of misdiagnosis of uterine sarcomas.Methods A retrospective study of 36 cases of sarcomas admitted in our hospital between 1980 and 2001 was performed.Results Uterine sarcomas were easily misdiagnosed as leiomyoma.① 21 cases(58.3%,groupⅠ) were misdiagnosed at their first operation,of which 14 cases were reoperated and 5 underwent 3 times operations.A high recurrence rate of 70% was found.There were 15 cases (41.7%,groupⅡ) who were correctly diagnosed at their first diagnosis.②The median ages of groupⅠwere lower than those of group Ⅱ.The chances of recurrence and dissemination of tumor cells were high when the patients were misdiagnosed as uterine leiomyoma and then operations of partial resection were performed.③ The amount of bleeding and transfusion during reoperation in groupⅠ was significantly more than that in groupⅡ.Conclusions Correct diagnosis at the first time and complete resection of the primary tumor were very pivotal to improved prognosis of uterine sarcomas.
