环孢素结合蛋白AmRNA表达与移植肾急性排斥的关系 被引量:3

Relationship between cyclophilin A mRNA expression and acute rejection of human graft kidneys

摘要 目的 观察肾移植患者环孢素结合蛋白A(CyPA)mRNA表达水平与血环孢素(CsA)浓度和急性排斥的关系。方法 采用逆转录聚合酶链反应动态测定23例肾移植稳定患者和10例急性排斥患者的移植肾活检标本及外周血淋巴细胞中 CyPA mRNA的表达。结果 应用CsA后肾功能稳定患者,CyPA的表达水平,在淋巴细胞(P=0.013)和肾组织(P=0.038)均显著上升,而急性排斥患者则无明显变化(P>0.05);淋巴细胞中 CyPA的表达与 CSA浓度存在低度相关性(r=0.384,P=0.047);急性排斥患者淋巴细胞中 CyPA表达水平低于肾功能稳定者(P=0.009)。结论 应用 CSA预防排斥的肾移植患者淋巴细胞中CyPA低表达是导致排斥发生的可能因素之一。对淋巴细胞CyPA表达水平进行监测,并结合CSA血浓度,有可能指导CSA的合理应用。 Objective To observe the expression level of cyclophilin A(CyPA) mRNA in kidney tissue and peripheral blood lymphocyte of renal transplant patients, and research its relationship with acute rejection and blood CsA concentration. Methods CyPA mRNA extracted from biopsy tissue of graft kidneys and anti-coagulated blood from 23 stable patients and 10 cases of acute non-humoral rejection, was measured by reverse-transcription polymerase chain reaction. Results Enhancing CyPA expression of stable patients after using CsA occurred in peripheral lymphocytes (from 0.53±0.66 to 1.04±0.88, P=0. 013, n =21) and kidney tissues(from 0. 62±0. 16 to 0. 79 ±0. 26, P = 0. 038, n= 6), while it was not found in acute rejection patients( P > 0. 05) . CyPA expression level of lymphocytes was slightly correlated with blood CsA concentration( r=0. 384, P = 0. 047) . CyPA expression level in lymphocytes of the rejection patients decreased significantly as compared with stable patients (0. 40 ±0. 22 vs 1. 04 ±0. 88, P = 0. 009) Conclusions CyPA expression level in peripheral blood lymphocytes may be one of the factors causing acute rejection. Monitoring the CyPA expression level in lymphocytes and the blood CsA concentration may be effective for guiding the reasonable usage of CsA.
