摘要
目的 探讨在神经导航指导下 ,脑肿瘤及周围结构的定位和外科手术治疗效果 .方法 应用 Stealth Station和Vector Vision神经导航系统 ,手术切除 36例脑肿瘤 ,总结治疗效果 ,分析定位准确性与疗效的关系 .结果 36例脑肿瘤共 36个 ,全切除 35个 (97% )、次全切除 1个 (3% ) .出院时患者症状明显改善 2 9例 (81% )、无明显变化 7例 (19% )、本组无症状恶化及死亡 .平均注册误差和 10 cm预期准确性分别为 (2 .0 9± 0 .6 7) mm和 (1.95± 0 .37) mm.术中持续准确性SA1,SA2和 SA3分别为 (2 .0 2± 0 .95 ) ,(1.0 3± 0 .48)和 (1.15± 0 .98) mm.结论 使用 Stealth Station和 Vector Vision神经导航系统 ,在术前可制定脑肿瘤手术计划 ,术中提供瘤体的动态跟踪和实时导航 ,手术定位准确、可靠 ,有助于肿瘤全切除及降低手术并发症 。
AIM To locate brain tumors and their peri structures by the guide of neuronavigation system and elucidate surgical effects. METHODS 36 patients with intracranial tumors were surgically treated with Stealth Station and Vector Vision system. The treatment effects were summarized and the neuronavigational accuracy with different patients were discussed. RESULTS All 36 patients revealed 36 brain tumors. Total tumor removal was achieved in 35 cases (97%), subtotoal removal in 1 case (3%). The neurological functions were improved in 29 cases (81%), unchanged obviously in 7 cases (19%). No case died. Mean fiducial error (MFE) and predicted accuracy at 10 cm (PA) were ( 2.09 ±0.67) mm and (1.95±0.37) mm respectively. Sustained accuracy including SA1 (K wire), SA2 (after craniotomy), and SA3 (after tumor removal) were ( 2.02±0.95), ( 1.03 ± 0.48 ) and (1.15±0.98) mm respectively. CONCLUSION The navigation system is reliable and accurate in making crania cerebral neurosurgical plans before operation. It can provide dynamic tracing of the tumor in the surgical operation and guide its manipulating. The technology helps total removal of tumors and reduces postoperative complications.
