摘要
目的 制作激素性骨坏死的动物实验模型 ,详细观察骨坏死组织病理学变化 ,探讨激素性骨坏死的发病机制。方法 采用 3 4只健康成年中国大耳白兔 ,随机分成单纯激素组 (A组 ,5只 ) ,单纯马血清组 (B组 ,10只 ) ,联合使用马血清和激素组 (C组 ,14只 )及正常对照组 (D组 ,5只 ) 4组。各实验组于激素注射后 4周处死 ,取内脏及骨标本 ,作组织病理学观察。结果 C组骨坏死发生率较高 ( 71 4 3 % ) ;使用激素的动物肝细胞脂肪变性 ,髓腔内脂肪细胞直径增大 ;坏死的股骨头软骨下可见髓腔内脂肪细胞明显增大 ;骨细胞内脂滴存在 ,骨内血管内皮细胞空泡样变性 ;软骨下骨小管内见血栓形成。结论 激素导致的脂质代谢紊乱、脂肪细胞改变可能是非创伤性骨坏死的初始病变 。
Objective To study the pathogenesis through observing histopathological changes in animal model of steroid induced osteonecrosis.Methods Thirty four adult health rabbits were randomly divided into four groups:Group A treated with dexamethasone consisted of 5 rabbits.Group B received sterile horse serum intravenously was made up of 10 rabbits.Group C treated with a combination of horse serum and dexamethasone consisted of 14 rabbits.Group D comprised 5 control rabbits.All animals were killed 4 weeks after their final treatment,and the liver,heart,kidney,lung and bone samples were obtained to observe the pathologic changes.Results The incidence of osteonecrosis was highest (71 43%) in Group C.Fatty degeneration of hepatocyte and the enlarging of diameter of fat cell was observed in the animals treated with steroid.Evidence of fat cell hypertrophy and some thrombosis were observed in the subchondral region of the femoral head,and lipid droplets were also seen in the osteocytes of the femoral head of these animals under electron microscope.Conclusions The histopathological changes of fat cell is likely to be the primary changes of the osteonecrosis,and the direct cause leading to osteonecrosis may be the thrombosis in the intraosseous micro circulation.
