摘要
目的 分析成人腹型过敏性紫癜的临床特点与内镜表现 ,为该病早期诊断提供依据。方法 对 13例成人腹型过敏性紫癜的临床特点与内镜表现进行了分析。结果 13例成人腹型过敏性紫癜均有阵发性腹部绞痛 ,恶心呕吐 9例 ,血便 2例。胃镜检查发现十二指肠粘膜有散在的大小不一的出血斑 ,病理检查见粘膜肌组织内大量中性粒细胞及淋巴细胞浸润。腹痛发生后 2～ 7天出现下肢紫癜。结论 对于非急腹症腹痛病人鉴别诊断时 ,需早期进行胃镜检查 。
Objective To eveluate the clinical and the endoscopic manifestation of purpura abdominalis in adult patients so that to reach the diagnosis early.Methods The endoscopic and clinical manifestation of 13 patients with purpura abdominalis were studied.Results All the patients complained of paroxysmal abdominal colics. Two patients complained of hematochezia and 9 complained of nausea and vomiting. Endoscopy found bleeding spots scattered in duodenal mucosa. Pathology study showed a lot of neutrophils and lymphocytes infiltration in mucosa muscular tissues.2～7 days after, pliosis were found on the shanks. Conclusion Early endoscopy is important in the differential diagnosis of purpura abdominalis.
