摘要
目的了解超广谱 - β内酰胺酶 (ESBL s)在本地区中小医院的流行及耐药情况。方法纸片扩散确认试验和双纸片协同试验 ,药敏试验采用纸片扩散法。结果 195株大肠埃希菌和 16 2株肺炎克雷伯菌确认试验阳性率分别为 32 .8%和 30 .9% ,双纸片协同试验与确认试验符合率分别为 92 .2 %和 90 %。产 ESBL 菌对头胞他啶、头胞曲松、头胞噻肟的耐药率从 38%~ 10 0 % ,而不产 ESBL 菌耐药率仅为 0 %~ 4.6 % ,产 ESBL 菌对氨基糖苷类、氟喹诺酮类、磺胺类具有多重耐药性 ,耐药率为 43.8%~ 10 0 %。结论基层临床实验室应尽早开展 ESBL 的检测 ,有效控制 ESBL
Objective To study the epidemic and drug resistant condition of extended spectrum beta lactamases in Grade II Hospital. Methods Disc diffusion confirmatory test and double disc synergy test were used. Disc diffusion test was used in drug resistant test. Results The positive rate of 195 Escherichia coli and 162 Klebsiella pneumoniae in confirmatory test were 32 8 % and 30 9 %,respectively. The identical rate of double disc synergy test and confirmatory test were 92 2 % and 90 %. The drug resistant rate of the bacteria which produce ESBL to ceftazidime, cefotaxime and ceftriaxome ranged from 38 % to 100 % while the bacteria with no ESBL producing got the drug resistant rate only 0%~4 6 %. The bacteria producing ESBL had multiple drug resistance to aminoglycosides, quinolones and sulfonamides, the drug resistant rate ranged from 43 8 % to 100 %. Conclusion ESBL detection test shoud be used in the basic clinical microbiological laboratory to control the ESBL spread and epidemic.
