引起暂时性阈移的声暴露敏感因素的耳声发射测试对比研究

摘要 目的 寻找早期发现噪声性听觉损害的敏感参数 ,明确可引起暂时性阈移的最敏感暴露诸因素及其交互作用。方法 正交方法观察 0 5、 1、 4kHz窄带噪声及 1kHz纯音 ,85、 10 0、 115dBSPL暴露强度 ,2 5、 4 5、6 5min暴露时程耳声发射变化 ,并与 0 2 5~ 8kHz各倍频纯音听阈、 0 5~ 4kHz各倍频及BBN镫骨肌反射阈相对照。结果 三因素中强度最为主要 ,85dBSPL时多数观察对象出现各项听功能指标出现改变并随强度加强而更明显 ;频率和暴露时间作用次之。 1kHz的纯音暴露后 ,各项听功能指标的改变相对较大 ,4 5分钟的暴露时间 ,听功能改变相对较大。结论 引起暂时性阈移的最佳因素组合是 1kHz、 115dBSPL、 6 5分钟 ;引起较为稳定的暂时性阈移的最佳因素组合是 1kHz、 10 0dBSPL、 4 5分钟 ; Objective To look for the sensitive factors that can induce noise-induced transient threshold shift(TTS).Method The Right-Cross Designs were used to observe the effect of different exposure conditions including 4 tone types(0.5, 1, 4kHz narrow noise, 1kHz pure tone), 3 intense levels(85, 100, 115 dB SPL)and 3 exposure times(2 5, 4 5, 6 5 minutes)on the otoacoustic emissions(OAEs)after TTS occurred. The indices were TEOAE response and wave repro, DPOAE amplitude, contrasted by audiogram of 0 25~8kHz, acoustic reflex of 0 5~4kHz and BBN. Results The result showed that the level of exposure was the most important factor. 85dB SPL acoustic exposure could induce DPOAE amplitude and changes of other testing indices. The higher level was, the biger change occurred. The tone type and time of exposure were shown less effects on TTS. Most of the indices showed more changes after 1kHz pure tone and 4.5 mimutes acoustic exposure. Those subjects who received 100 dB SPL or 115dB SPL acoustic exposure showed indices changes aft er 20 minutes. Conclusion The result suggests that the best combination of the three factors for inducing TTS is 1kHz pure tone, 115dB SPL, and 6.5 minutes; the best combination for inducing stable TTS is 1kHz pure tone, 100dB SPL, and 4 5 minutes; to find the hearing loss, the best method is the measurement of DPOAE amplitude.
