摘要
目的 矫正鼻唇沟过深、颌下线中断等面中下部老化表现。方法 行面中下部超过鼻唇沟的广泛皮下及SMAS下剥离 ,将掀起的SMAS瓣 ,依耳轮脚及耳垂下极水平分成三份 ,分别向上、后、乳突方向悬吊 ,对松弛的眼轮匝肌进行紧缩和悬吊 ,将颧脂肪垫向外上方向牵拉固定于颧骨体表面筋膜 ,去除部分过度增厚的颧脂肪垫。结果 12例面中部老化明显者 ,手术后随访 5个月至 3年 ,效果良好 ,无面瘫、血肿等并发症。
Objective To adequately correct the middle and lower face aging appearances such as prominent nasolabial fold and broken jawline. Methods A sub-SMAS and subcutaneous dissection was extended superiorly over the zygomaticus muscle and medially beyond the nasolabial folds. The SMAS flap was divided into three segments according to crus of helix and ear lobe then advanced and fixed in supero-posterior, posterior and mastoid process direction respectively. The ptotic orbicularis oculi muscle was tightened and advanced. The malar fat pad was lifted and repositioned along the appropriate superolateral direction with a suspension suture at the malar eminence to the superolateral fascia, sometimes part of the too large malar fat pad was excised. Results The authors did rhytidectomy in 12 cases by this technique with impressive and minimal complications(hematoma, facial paralysis). Conclusion This method is a safe and effective way to individuals with significant middle and lower face aging appearance.
