摘要
目的 :探讨TSGF(TumorSuppliedGroupofFactors)和癌抗原 1 2 5(CA1 2 5)联合诊断卵巢恶性肿瘤的价值。方法 :同时测定 83例盆腔包块患者血清TSGF和CA1 2 5水平 ,其中术后病理证实为卵巢癌 30例、卵巢良性肿瘤 37例和盆腔子宫内膜异位囊肿 1 6例 ,分析这两个指标的临床价值。结果 :①卵巢癌组TSGF、CA1 2 5水平分别为 ( 79.6 2± 1 3.58)U ml,( 2 51 .2 3± 2 72 .48)U ml,明显高于内膜异位症组 [( 58.47± 1 4.51 )U ml,( 6 9.76± 1 0 9.73)U mlP <0 .0 1 ) ]和卵巢良性肿瘤组 [( 6 4 .0 7± 7.0 2U ml) ,( 2 5.2 9± 31 .6 3)U mlP <0 .0 1 ) ];内膜异位症组CA1 2 5 明显高于良性肿瘤组 (P <0 .0 1 ) ,两组TSGF水平无差异。②卵巢恶性肿瘤组TSGF、CA1 2 5 阳性检出率( 6 3.33%、76 .6 7% )明显高于良性肿瘤组 ( 1 8.92 %、7.1 4% ,P <0 .0 1 )和内膜异位症组 ( 2 5.0 0 %、31 .2 5% ,P <0 .0 1 )。③TSGF和CA1 2 5对卵巢癌的诊断敏感率为 6 3.33%和 76 .6 7% ,而二者联合检测敏感率达 90 .0 0 %。结论 :TSGF的检测对卵巢恶性肿瘤的诊断有辅助作用 ,且能弥补CA1 2 5诊断内膜异位症的不足 ,有利于盆腔肿块的鉴别 ;TSGF和CA1 2 5联合检测能明显提高卵巢癌的诊断敏感性。
Objective:To investigate the diagnostic value of TSGF(Tumor Supplied Group of Factors) and CA 125 on malignant ovarian neoplasm.Methods:Determine the levels of serum TSGF and CA 125 in 30 women cases of ovarian cancers,37 cases of benign ovarian tumor and 16 cases of endometrioses. Compare the diagnostic sensitivity, specificity and coincident ratio of two parameters. Results:The levels of TSGF and CA 125 of malignant group[(79.62±13.58) U/ml,(251.23±272.48) U/ml]are significantly higher than these of the endometriosis group[(58.47±14.51) U/ml,(69.76±109.73) U/ml, P <0.01)]and the benign group[(64.07±7.02) U/ml,(25.29±31.63) U/ml, P <0.01)];The level of CA 125 of malignant group is significantly higher than that of the endometriosis group( P <0.01),but the level of TSGF is not significant in the 2 groups. The positive rates of TSGF and CA 125 in malignant group(63.33%,76.67%)are significantly higher than those in the benign group (18.92%,7.14%, P <0.01)and in the endometriosis group(25.00%,31.25%, P <0.01).The sensitivities of TSGF and CA 125 to malignant ovarian tumor are only 63.33 % and 76.67%,but it will be up to 90.00% with 2 parameters at the same time. Conclusions:The determination of TSGF can be an auxiliary aid in diagnosing ovarian cancers, and can also make up the defect of CA 125 using to distinguish pelvic endometriosis and ovarian cancers; The determination of TSGF together with CA 125 can obviously improve the sensitivity to ovarian cancers.
