摘要
目的 :研究酒精性肝硬化 (Alcoholiclivercirrhosis ,ALC)的临床特点。方法 :对我院 1993年 1月～ 2 0 0 0年 6月住院的 5 3例ALC患者与同期住院的 5 2 2例肝炎后肝硬化 (virallivercirrhosis ,VLC)患者进行对比分析。结果 :ALC和VLC的腹胀、纳差、乏力、腹水和脾大等表现无显著性差异 (P >0 0 5 )。肝掌、蜘蛛痣、男性乳房发育、肝大和黄疸ALC较VLC常见 (P <0 0 1)。ALC患者的AST/ALT、r -GT、ALP、TBil明显高于VLC(P <0 0 1或P <0 0 5 )。结论 :ALC和VLC的临床表现有较大的差异。肝掌、蜘蛛痣、男性乳房发育、肝大和黄疸在ALC中常见 ,且AST/ALT、r GT、ALP、TBil的水平高于ALC。掌握这些特点对ALC的诊断及鉴别诊断有重要的参考价值。
Objective: To study the clinical characteristics of alcoholic liver cirrhosis.Methods: From Macth 1993 to Sixth 2000, clinical manifestations of 53 patients with alcoholic liver cirrhosis were analyzed and compard with 533 patients of viral liver cirrhosis.Results: Clinical symptoms of alcoholic liver cirrhosis and viral liver cirrhosis were mainly flatulence, anorexia and fatigue. There is no difference between them ( P >0 05).Spider angiomata,palmar erythema, gynecomastia, hepatomegaly and jaundice were more frequent in patients with alcoholic liver cirrhosis than patients with viral liver cirrhosis( P <0 01).AST/ALT,r GT,ALP and TBil in patients with alcoholic liver cirrhosis were obviously higher than in patients with viral liver cirrhosis( P<0 01 or P<0 05 ).There is no differene in A/G between them( P>0 05 ).Conclusion: The incidence of spider angiomata,palmar erythema, gynecomastia, hepatomegaly and jaundice in patients with alcoholic liver cirrhosis were more frequents in patients with alcoholic liver cirrhosis, the levels of AST/ALT,r GT,ALP and TBil is obviouslg higher than patients with viral liver cirrhosis. It is of value to study the clinical characteristics in making a diagnosis and differential diagnosis.symptomsofalcoholicliv
