摘要
目的 :探讨 2型糖尿病伴高血压和血压正常时胰岛素作用和分泌的变化。方法 :2 98例 2型糖尿病分为高血压组95例和血压正常组 2 0 3例 ,比较两组口服葡萄糖耐量试验 (OGTT)和胰岛素释放试验中血糖及胰岛素 ,并以稳态模型 (Homa model)评估公式胰岛素抵抗指数 (IR)、β细胞功能指数 (HBCI)及胰岛素敏感指数 (IAI)评价胰岛素抵抗和 β细胞功能。结果 :高血压组患者空腹血糖、空腹胰岛素显著高于血压正常组 (P<0 .0 1和 P<0 .0 5 ) ,与 IR呈显著正相关 (P<0 .0 1) ,与 IAI和 HBCI呈负相关 (P<0 .0 5 )。各组内肥胖者亦显示与 IR明显正相关而与 IAI负相关 ,P<0 .0 1。说明高血压组患者胰岛素抵抗显著 ,β细胞分泌功能下降 ,伴有肥胖者胰岛素抵抗更加显著。结论 :2型糖尿病伴高血压患者存在显著的胰岛素抵抗及 β细胞分泌功能紊乱 ,肥胖加重了胰岛素抵抗。
Objective:To study the changes of insulin seretion in diabetes mellitus Ⅱ with and without hypertension. Methods:298 cases of diabetes mellitus Ⅱ were divided into two groups according to their blood pressure. 95 cases were selected in hypertension group, and 203 cases were in control group. The oral glucose tolerant tests (OGTT) and insulin releasing test were performed to compare the levels of glucose and insulin in two groups.Homa model were used as IR,HBCI and IAI index,to evaluate insulin resistance and β cell function. Results:The fasting blood suger and insulin levels in hypertension group were significant higher than those in control group( P <0.01). The obesity patients in the two groups showed a significant insulin resistance as compared to those of non obesity patients ( P <0.01).Conclusion :There is a significant insulin resistance and disorder of β cell function in diabetes mellitus Ⅱ with hypertension. The obesity may increase the insulin resistance.
