摘要
目的 :对荧光定量 PCR(FQ- PCR)测定的 10 2例 HBV- DNA结果进行分析 ,以探讨其临床价值。方法 :对 10 2份临床血清标本用 FQ- PCR方法进行 HBV- DNA定量测定 ,并和 EL ISA两对半结果以及 AL T结果进行比较分析。结果 :2 5份 HBs Ag(+)、HBe Ag(+)、HBc Ab(+)的标本 HBV- DNA均为阳性 ,L og DNA平均值± 2 sd为 7.45± 2 .5 4;34份 HBs Ag(+)、HBe Ab(+)、HBc Ab(+)的标本 HBV- DNA阳性 15份 ,L og DNA平均值± 2 sd为 6 .33± 1.86 ;9份HBs Ag(+)、HBc Ab(+)的标本 HBV- DNA阳性 4例 ,L og DNA平均值± 2 sd为 5 .2 3± 4.6 0 ;14份 HBs Ab(+)的标本 HBV- DNA阳性 1例 ,18份全阴性的标本 HBV- DNA无阳性。以上五种两对半结果其相对应的 HBV- DNA值基本呈下降趋势 ,而且前二者之间有显著性差异 (P<0 .0 1)。急性乙型病毒性肝炎、慢性乙型病毒性肝炎、肝癌、肝硬化患者之间的 HVB- DNA定量结果无显著性差异。HBV- DNA和 AL T之间无相关性 (r=0 .17)。结论 :FQ- PCR定量测定HBV- DNA可以真实反应 HBV的感染和复制情况 ,可以用于临床治疗和疗效观察 ,但它在急性乙型病毒性肝炎、慢性乙型病毒性肝炎、肝癌、肝硬化的鉴别诊断上无帮助 ,和病情的轻重也无相关性。
Objective:To evaluate the clinical significance of fluorescence quantitative PCR (FQ PCR) on detection of type B hepatitis. Methods:The type B hepatitis DNA (HBV DNA) was measured by FQ PCR in 102 serum samples and compared them with the results from ELISA and ALT.Results:In 25 HBsAg(+)/HBeAg(+)/HBcAb(+) samples,HBV DNA results were all positive,the average of LogHBV DNA was 7.45±2.54. In 34 HBsAg(+)/HBeAb(+)/HBcAb(+) samples,the HBV DNA results of 15 samples were positive,the average of LogDNA was 6 33±1 86. No differences of HBV DNA quantity were found among the patients of acute type B hepatitis, chronic type B hepatitis, hepatocarcinoma and cirhosis. No correlation between the HBV DNA and ALT was found r =0 17. Conclusion:The detection of HBV DNA by FQ PCR reflects the situation of infection and replication of HBV and it can be used in the clinical practice. There are no any diagnosis significance among acute and chronic type B hepatitis, hepatocarcinoma and cirhosis while using FQ PCR.
