类风湿关节炎中HLA-DR分子表达量的改变 被引量:3

摘要 目的 探讨外周血淋巴细胞表面HLA DR分子表达量在类风湿关节炎 (RA)发病中的作用。方法 选择RA患者活动期 34例 ,静止期 40例 ;正常对照 (NC) 4 3例 ,急性风湿性关节炎(ARA) 2 1例及系统性红斑狼疮 (SLE) 2 8例 ,分别检测外周血淋巴细胞表面的HLA DR分子表达量。同时在活动期RA治疗 3个月前后 ,分别检测HLA DR分子表达量、类风湿因子 (RF)及对症状计分 ,进行统计学分析。结果 ①RA活动组HLA DR分子表达水平最高 (P <0 0 5 ) ,ARA组及RA静止组较NC组明显增高 (P <0 0 5 ) ,而SLE组与NC组无明显差别 (P >0 0 5 ) ;②RA活动组HLA DR分子表达量及RF的阳性率均在治疗后显著下降 ,临床症状明显改善。结论 ①检测患者外周血淋巴细胞表面HLA DR表达量有助于RA的诊断及监测RA活动度 ;②HLA DR分子表达量与RF的动态变化相平行 ;③动态观察HLA Objective To explore the effect of the expression level of HLA DR on the surface of lymphocytes of peripheral blood on the onset of rheumatoid arthritis (RA).Methods The research involved 34 cases of active RA,40 cases of static RA,21 cases of acute rheumatic arthritis (ARA),28 cases of systemic 1upus erythematosus (SLE) and 43 cases of normal people as control group (NC).The expression levels of HLA DR on the surface of lymphocytes were detected.The expression level of HLA DR,rheumatoid factor and symptom scores were examined before and after 3 month treatment in active RA group.The data were analyzed.Results ① The expression levels of HLA DR in active RA group were the highest among all groups ( P <0 05).The expression levels of HLA DR were significant higher in static RA group and ARA group than in control group ( P <0 05). There was no significant difference between SLE and control group on the expression level of HLA DR ( P >0 05).② The expression level of HLA DR and positive rate of rheumatoid factor both decreased significantly in active RA groups ( P >0 05) after treatment.At the same time,the clinical symptoms were improved significantly.Conclusion ① Testing expression level of HLA DR is helpful to diagnose and monitor the state of RA.② The change of expression level of HLA DR conforms to that of rheumatic factor.③ Examination of the change of expression level of HLA DR can help to judge the state and prognosis of RA.
