摘要
目的探讨一种既能充分减压又能维持腰椎稳定的治疗腰椎管狭窄的手术方法。方法32例60岁以上的病人采用保留棘突和部分椎板的椎管减压术,根据术后症状改善程度来评价减压的彻底性,根据术前术后动态X片来评价腰椎稳定性。结果手术后症状完全改善29例,优良率为90.6%,术后发生椎体滑脱3例,占9.3%。讨论保留棘突和部分椎板的椎管减压术是一种既能充分减压,又能达到椎体稳定的治疗腰椎管狭窄的手术方法。
Objective To evaluate the result of the spinal c anal decompression by partial lamin ectomy with preservation of the spinous process for stenosis of t he lumbar spinal canal.Methods From 1993through 1997,32patients with age over 60were subjected to this operation.The sym ptoms and the roentgenography of the lumbar spinal canal before and after the operation were compared.Results Postoperatively,the symptoms comp letely disappeared in 25patients,p artially relieved in 6,and persiste d in1.X -ray disclosed that spondylolithesis became worse in only one case after the operation.Conclusions Partial laminectomy with preservation of the spinal proc ess is an effective operative proced ure for the treatment of stenosis of t he lumbar spinal canal in the elderly.The stability of the lum bar spine is not inevitably compromi sed.[
