全麻开胸术后患者痰粘弹性变化 被引量:41

Change of Sputum Viscoelasticity in Patients Undergoing General Anesthesia and Thoracotomy

摘要 目的 进一步了解全麻开胸患者术后痰液粘弹性变化,为探索更理想的雾化湿化方法及排痰措施奠定基础。方法 测量68例患者术后第1~5天痰液屈服应力、粘性、弹性等流变学指标。结果 术后第1~5天,痰屈服应力呈逐渐降低趋势,由11.3Pa降至4.7Pa,有显著性差异(P=0.0001),第二天显著大于第 2~4天。术后第 1~5天,痰液粘性呈逐渐下降趋势,不同时间有显著性差异(P=0.0013),第1天(0.92Pa.s)显著大于第4天、第5天(0.27Pa.s,0.29Pa.s)。术后第1天弹性(1.12Pa.s)显著高于术后第2~5天(0.29Pa.s~0.049Pa.s),有显著性差异(P=0.0070)。结论 全麻开胸患者术后第1天痰粘弹性显著增高,导致患者排痰困难,应进一步探索更理想的祛痰排痰措施。 Objective The study is to further understand the sputum viscoelasticity changes in pa-tients after general anesthesia and thoracotomy. Methods 68 patients after thoracotomy were in-volved to measure the sputum rheology. Results Sputum yield stress was significantly decreased from day 1(11. 3 Pa) to day 5(4. 7 Pa). Viscosity on the first postoperative day(0. 92 Pa. s) was significantly higher than that on the fourth and fifth day(0. 27 Pa. s,0. 29 Pa. s). Elasticity was also significantly higher on day 1(1. 12 Pa. s)than that on day 2~5(0. 29 Pa. s~0. 04 Pa. s). Conclusion Patients after thoracotomy could not expectorate smoothly owing to high yield stress and vis-coelastisity of sputum on the first postoperative day. More effective mucokinetic measures should be developed.
