摘要
目的 分析急性心肌梗死（ＡＭＩ）介入和非介入治疗的临床疗效、费用和护理差异以及护理要点，并比较美国同类医院的相关资料及其差别。方法３所医院１９９８年１月至２０００年７月住院治疗的５４例急性心肌梗死患者分为两组，介入２１例，非介入３３例，分析两组的临床特点、临床疗效、住院费用和住院时间以及临床护理的差异。Ｃｈｉ－Ｓｑｕａｒｅ和 Ｓｔｕｄｅｎｔ ｔ Ｔｅｓｔ比较各组之间的差异，以Ｐ＜０．０５为相差显著。结果 两组间临床疗效无明显差异，但介入治疗组患者的平均住院时间和住院费用均明显高于非介入组（Ｐ＜０．０５）。介入治疗组的护理内容比非介入组复杂，强度和难度都比较高。结论 介入治疗和非介入治疗的临床疗效与美国同类医院类似，住院时间相对较长，虽然介入治疗住院时间长于非介入治疗的趋势一样；介入治疗费用相对较高，而美国同类医院介入和非介入治疗费用差距不大，这与介入治疗开展广泛程度和医疗仪器设备收费有关。护理内容和难度没有差异。
Objectives The outcome, in-hospital direct cost and nursing of acute myocardial infarc-tion (AMI) patients treated by invasive and noninvasive methods in China were studied. The re-sults were compared with the data obtained from hospitals in USA under the similar conditions. Methods We analyzed 54 patients from our 3 affiliated hospitals from January 1998 to July 2000. The date collection included clinic manifestations, clinic outcomes, in-hospital direct cost and length of hospital stay by invasive treatment (including coronary angioplasty, or percutaneous transluminal coronary angioplasty PTCA) and noninvasive treatment. Chi-Square and Student t test were used for statistical analysis. Results The clinic outcomes showed no difference between the two groups, but the direct cost was higher and length of hospital stay was longer in invasive group as compared with noninvasive group ( P <0. 05). The nursing process of invasive group was more complicated and difficult than that of noninvasive group. Conclusion The hospital outcome from both invasive and noninvasive group was the same, similar to the report from US hospitals with the same conditions. The length of hospital stay was longer in invasive group than that in non-invasive group in both reports;the average length of hospital stay in China was longer than that of US hospitals. When hospital direct cost was calculated, the invasive group cost more than nonin-vasive group in China hospitals, but there was no significant difference between invasive and non-invasive groups in US hospitals. The reason may be that invasive method is more popular, and the equipment cost is relatively lower in US.
