摘要
目的 总结重型颅脑损伤救治经验 ,探讨建立创伤系统 (traumasystem)在重型颅脑损伤救治中的临床意义。方法 近 2 0年来收治重型颅脑损伤 4 673例 ,采取分期有重点的连续性治疗 ;其中近5年收治 13 81例 ,在分期治疗基础上 ,急性期采用本院建立的“颅脑创伤高级救治系统”救治。结果 2 0年总计 ,恢复良好率 2 5 .3 % ( 1182例 ) ,死亡率 4 1.5 % ( 193 9例 ) ;近 5年恢复良好率已升至 3 6.7% ,死亡率已降至 3 1.8%。结论 在“创伤系统”模式下 ,对重型颅脑损伤进行自院前现场即开始的加强性、连续性早期救治 ,最后在ICU环境下实施具体、目的性急性期治疗 ,注重康复期手术治疗 ,能降低死亡率、提高生存质量。
Objective To summ arize experience about the management an d treatment of severe cerebral injury,di scuss the clinical significance of esta blishing 'trauma system' on management of this disease.Methods 4 673 cases of sever e cerebral injury in recent 20 years wer e subjected to the continuing treatment that has focal point by stage,among whic h in recent 5 years on the foundation o f staging treatment 1 381 cases had bee n admitted and treated,adopting 'Advance d brain trauma system' established by ou r hospital during the acute stage.Results Total of recent 20 y ears:1 182 cases (23.5%)received good re covery,1 939 cases(41.5%)died.In recent 5 years the good recovery rate has risen to 36.7%,and mortality has fallen to 31 .8%.Conclusions Under the mo del of the trauma system continuing inte nsive management and treatment in early stage that begins at accident spot through emergency department for p atients with severe cerebral injury,and finally under ICU environment implementi ng specific objective acute stage treatm ent after admission,attaching importance to surgical treatment during rehabilitation ph ase can reduce mortality and rise surviv al quality.
