摘要
目的 气虚血瘀热郁是慢性萎缩性胃炎及胃癌前期病变的病理基础 ,制定益气活血清热方 ,设胃宁冲剂为阳性对照组 ,观察该方对慢性萎缩性胃炎及其癌前期病变的临床疗效。方法 治疗组 6 4例 ,对照组 6 3例 ,3个月为 1个疗程 ,服药 2个疗程。结果 治疗组 3个月、6个月临床有效率为 75 .0 %、95 .3 % ,均显著高于对照组的 5 2 .4%、6 3.5 % (P <0 .0 1) ;治疗组对胃镜、萎缩、肠化的疗效均显著高于对照组 (P <0 .0 5 ) ,而对炎症的改善作用 2组差异无显著性。治疗后2组症状积分比较 ,治疗组痞满、纳差、嗳气、便溏积分及总积分均显著低于对照组 (P <0 .0 5 ) ;治疗后治疗组萎缩、肠化、异型增生积分均较治疗前显著下降 (P <0 .0 1) ,对照组萎缩、肠化积分也较治疗前显著下降 (P <0 .0 5 ) ,而治疗组积分又显著低于对照组 (P <0 .0 5 )。
OBJECTIVE The pathological basis for chronic atrophic gastritis and precancerous pathological changes is qi deficiency, blood stasis and heat accumulation. The prescription for invigorating qi, activating blood circulation and clearing away heat is used in this study to observe its effect in the treatment of chronic atrophic gastritis and precancerous pathological changes, compared with a positive control group treated with Weining Granule. METHOD There were 64 patients in the treatment group and 63 patients in the control group. A course of treatment lasted for 3 months, and the patients were treated for two courses of treatment. RESULT In the treatment group, the effective rates after 3 and 6 months of treatment were 75.0% and 95.3% respectively, which were markedly higher than 52.4% and 63.5% in the control group ( P <0.01). There was no marked difference in the improvement of inflammation between the two groups. The total integral for the feeling of fullness, anorexia, belching and loose stool in the treatment group was markedly lower than that in the control group ( P <0.05). After the treatment, the integrals for atrophy, allotypical hyperplasia, etc. were markedly lower than those before the treatment in the treatment group ( P <0.01) and these integrals become lower also in the control group after the treatment ( P <0.05). Again, the integrals in the treatment group were markedly lower than those in the control group ( P <0.05). CONCLUSION The prescription for invigorating qi, activating blood circulation and clearing away heat is quite effective for the chronic atrophic gastritis and precancerous pathological changes.
