摘要
目的 :探讨老年人低能量损伤引起多发性骨折的临床特点。方法 :总结 1994年~ 2 0 0 0年收治的 2 2 6例 6 5岁以上患者多发性骨折的发病、诊断和治疗情况。结果 :2 2 6例中有 2处骨折 12 4例 (5 4.9% ) ,3处骨折 6 7例 (2 9.6 % ) ,3处以上骨折 35例 (15 .5 % )。以髋部合并腕部、髋部合并脊柱骨折为多。生活伤是主要的致伤原因 (85 .0 % )。临床延迟诊断及漏诊率为 17.3%。股骨Singh指数在Ⅰ°~Ⅲ°时 ,多发性骨折的危险性高。手术组平均卧床时间 2 3d。结论 :低能量的生活伤可导致老年人多发性骨折 ,Singh指数Ⅰ°~Ⅲ°时危险性更大。损伤部位以髋部合并腕部、脊柱骨折的发生率高。
Objective: To discuss the clinical features of multiple fractures caused by low-energy injury in elderly. Methods: Retrospective analysis was carried out in 226 elderly with multiple fractures. Results: There were two-site fractures in 124 cases, three-site fractures in 67 cases and more sites in 35 cases. The types were more common that were hip with wrist and hip with spinal fractures. The rates of delay diagnosis and missed diagnosis were 17.3%. It was higher risk of multiple fractures when the X-ray of proximal femoral showed Singh Index between Ⅰ°~Ⅲ°. Lying-bed period were 23 days in operation group. Conclusion: Low-energy injury could cause multiple fractures in elderly, especially when femoral X-ray showed Singh IndexⅠ°~Ⅲ°. The injury sites were more common in hip with wrist and hip with spinal than others. Surgicai operation could shorten the lying-bed times.
