摘要
①目的 初步探讨口腔扁平苔癣 (OLP)与幽门螺杆菌 (HP)的相关性。②方法 应用PCR技术检测30例OLP病人口腔多部位黏膜脱落细胞、牙结石及 19例病变组织中的HP DNA ,并与对照组进行比较。对 19例OLP病人病变组织做普通病理切片行苏木精 伊红染色及银染色 ,以观察HP入侵情况。③结果 15 0个黏膜脱落细胞样本中有 2 0个HP DNA阳性 ,与对照组比较差异有显著性 (χ2 =5 .13,P <0 .0 5 ) ;口腔各部位黏膜脱落细胞HP DNA阳性率以颊部最高 ,糜烂型与非糜烂型OLP病人HP DNA的检出率差异无显著性 (χ2 =0 .2 0 ,P >0 .0 5 )。19例病变组织中有 12例HP DNA阳性 ,与对照组比较差异有高度显著性 (P =0 .0 0 0 2 )。银染色切片可见到HP菌体。④结论 HP可在口腔黏膜上定居 ,OLP病变组织中有HP侵入 。
Objective\ To discuss the relationship between oral lichen planus (OLP)and helicobacter pylori(HP). Methods\ Polymerase chain reaction(PCR) was applied to detect HP in the oral mucosal epithelial cells and dental plaques of 30 OLP cases and 19 biopsy samples. 19 biopsy samples of OLP were examined by light microscopy with haematoxylin eosin (HE) and Warthin Starry silver (W S)stains.\ Results\ 20 were positive in 150 specimens for HP DNA by PCR in mucosal epithelial cells, the difference was significant compared with the control group ( χ 2=5.13,P <0.05) .The positivity rate was highest in buccal mucosa. There was no significant difference for HP DNA positivity rate between erosive OLP and non erosive OLP ( χ 2=0.20,P >0.05). 12 of 19 biopsy specimens were positive for HP DNA by PCR, the difference was significant compared with the control group ( P =0.000 2). HP was readily visible in sections with W S stain.\ Conclusion\ HP was proved to exist in oral epithelial cells of OLP patients and could invade into affected site of OLP. HP had relationship with certain OLP.
