摘要
目的 探讨Crose氏改良根治术治疗乳癌的疗效。方法 对采用Crose氏改良根治术 114例 (C组 ) ,Halsted根治术 2 0 5例 (H组 )的乳癌患者的临床资料进行回顾性分析和对比。结果 319例中 ,2 78例获随访 (C组 98例 ,H组 180例 ) ,随访 3～ 14年。随访 3年以上 2 78例 (C组 98例 ,H组 180例 ) ,5年以上 2 16例 (C组 5 6例 ,H组 16 0例 ) ,10年以上 6 9例(C组 31例 ,H组 38例 )。C组和H组 3,5 ,10年生存率分别为 10 0 % ,85 .71% ,70 .97%和 95 .6 % ,83.13 % ,71.0 5 % ;3 ,5 ,10年局部复发率为 4.0 8% ,7.1% ,6 .45 %和 3 .89% ,6 .88% ,5 .2 6 % ,3 ,5 ,10年远处转移率分别为 6 .1% ,12 .5 % ,12 .9%和 6 .6 7% ,11.2 5 % ,15 .79% ,两组各指标均无显著差异 (P >0 .0 5 )。而C组患者上肢活动功能较H组明显为佳 (P <0 .0 1)。结论 Crose氏改良根治术治疗乳癌 ,其根治性疗效同Halsted根治术 ,而患侧上肢功能好 ,并发症少 ,是一种有效。
Objective To study the effect of Crose's modified radical mastectomy on breast cancer (BC). Methods The clinical data of 114 cases of BC treated with Crose's modified radical mastectomy (group C) and 205 cases of BC treated with Halsted radical mastectomy (group H) were retrospectively analyzed and compared. Results Of 319 cases, 278 cases had been followed up for a period from 3 years to 14 years. All of the 278 cases were followed up for more than 3 years (group C 98 cases, group H 180 cases), 216 cases for more than 5 years (group C56 cases, group H160 cases), 69 cases for more than 10 years (group C31 cases, group H38 cases). In group C and group H, the 3-, 5-and 10-year survival rate were 100%, 85.71%, 70.97% and 95.6%, 83.13%, 71.05%, the 3-, 5-and 10-year local recurrence rate were 4.08%, 7.1%, 6.45% and 3.89%, 6.88%, 5.26%, the 3-, 5- and 10-year distant metastasis rate were 6.1%, 12.5%,12.9% and 6.67%, 11.25%,15.79% , respectively (P>0.05). The upper extremity function in group C was better than that in group H (P<0.01). Conclusions Crose's modified radical mastectomy for BC has similar effect as Halsted operation. Compared with group H, patients in group C have good function of upper extremity, and less complications. Crose's modified radical mastectomy for BC is an effective and satisfactory procedure.
