摘要
目的 探讨心肌肌钙蛋白T（cTnT）异构体的表达与心功能的关系。方法 取心脏外科手术患者左心室乳头肌或右心耳肌标本，行蛋白印迹检测cTnT蛋白质表达的变化。结果 对照组成人仅表达cTnT3；心功能Ⅱ级组1例不表达cTnT4，2例仅表达微量cTnT4，1例表达较高水平的cTnT4；心功能Ⅲ级以上组既表达cTnT3又表达cTnT4，1例心功能Ⅳ级的左房粘液瘤患者还表达cTnT2。 结论 心力衰竭时存在cTnT异构体表达的变化，胚胎型cTnT重新表达。
Objective To study the relation of the changes in the expression of cardiac troponin T (cTnT) isoforms with the cardiac function of patients with congestive heart failure. Methods Samples of the left ventricular papillary muscles and right atrial muscles were obtained from patients with congestive heart failure during cardiac surgeries and from healthy postmortem donors. Western blotting was employed to examine the expression of cTnT fetal isoforms in the samples. Results In the heart tissues of all normal subjects, only cTnT3 expression was detected, while in contrast, only 1 patient was found with cTnT3 expression in the 4 patients categorized into New York Heart Association (NYHA) class Ⅱ heart failure. Both cTnT3 and cTnT4 were detected in patients with NYHA class Ⅲ heart failure, and in addition, cTnT2 was found in 1 patient with left atrial mucoid carcinoma and with class Ⅳ heart failure. Conclusion In patients with congestive heart failure, the cTnT isoforms show a shift towards more fetal patterns, and cTnT4 expression is elevated with the deterioration of cardiac function.
