垂体微腺瘤MRI诊断(附25例分析) 被引量:4

摘要 目的 :探讨和总结 2 5例垂体微腺瘤的磁共振成像 (MRI)表现。方法 :对 2 5例经MRI平扫 :冠状位下SE序列T2 加权成像 (T2 WI)、FFE序列T1 加权成像 (T1 WI) ,及行Gd DTPA增强扫描 :冠状位下FFE序列T1 WI、矢状位下FFE序列T1 WI检查 ,经手术切除得到病理证实的病例进行分析 ,其中 7例行动态增强和延迟扫描。结果 :2 5例垂体微腺瘤均位于垂体前叶 ,单发 1 9例 ,其中 1 4例位于垂体右侧部 ,多发 6例。MRI平扫 2 4例 (T1 WI)为低或稍低信号、1例为高或稍高信号 ;2 3例 (T2 WI)为高或稍高信号 ,但 2例为等或稍低信号 ,增强后即刻扫描 2 5例均为低或稍低信号 ,7例动态增强扫描延迟 2 5~ 30min后 1例有明显强化。结论 :垂体微腺瘤MRI表现有一定特征性 。 Objective:To study the MR imaging features of 25 cases of pituitary microadenomas.Methods:25 cases of pituitary microadenomas confirmed by surgery and pathology were performed plain and Gd DTPA enhanced MR imaging,7 of which added to dynamic contrast enhanced and delayed enhanced MRI.Plain MR imaging was done using FFE sequence on T 1WI and SE on T 2WI at coronal plane.Enhanced MRI was taken using T 1WI FFE sequence at coronal and sagittal planes.Results:All pituitary microadenomas located in anterior lobe.19 of 25 cases had a single lesion,14 of which located in the right side of pituitary gland.In other 6 cases there were multiple lesions. T 1WI showed hypo or slight hypointensity in 24 cases and only one case showed hyper or slight hyperintensity.T 2WI demonstrated hyper or slight hyperintensity in 23 cases but iso or slight hypointensity in 2 cases.After enhancement all the lesions appeared hypointense,and only 1 of 7 cases with dynamic contrast enhancement showed dramatic enhancement after a delay of 25~30 minutes.Conclusion:Pituitary microadenomas have some characterictic MRI features.Moreover, MRI might play an important role in the diagnosis of the pituitary microadenomas.
