外周动静脉同步换血电解质和血糖变化的研究

摘要 目的 :探讨外周动静脉同步换血前后电解质和血糖的改变。方法 :对 2 5例高胆红素血症患儿采用同型进行外周动静脉同步换血 ,换血前后在同一动脉端取血测电解质和血糖。结果 :1换血过程中应用钙剂组 (观察组 )与未应用钙剂组 ,血钙水平在换血前后及两组之间均无显著差异。 2换血时静注高渗糖组糖换血后显著升高 ,而对照组无明变化。 3换血后血钾下降 ,血钠上升 ,但仍在正常范围。结论 :外周动静脉换血疗法对机体生理功能及电解质的平衡无明显影响 ,换血过程中静注钙剂是不必要的。 Objective: To investigate the changes in serum electrolytes and blood glucose during synchronous exchange transfusion via peripheral vessels. Methods: synchronous exchange transfusion of homotypic blood was performed via peripheral vessels in 25 cases of hyperbilirubinemia. Blood samples were taken through the identical point of the same artery immediately before and after exchange transfusion for determination of serum electrolytes and blood glucose. Results: (1) There was no significant change in serum calcium between the group administered calcium gluconate and the group not administered, nor was there significant change in both groups before and after exchange transfusion. (2) Blood glucose concentration was significantly elevated in the group administered hypertonic glucose during exchange transfusion but changed slightly in control group. (3) Na+ elevated and K+ descended after exchange transfusion, but within normal scale. Conclusion: There was no obvious change of electrolytes and physiologic function after exchange transfusion via peripheral vessels and the administration of calcium gluconate is not necessary during exchange transfusion.
