摘要
目的 探讨p2 1WAF1和LRP在乳腺癌中的表达及其预后意义。方法 运用免疫组化S P法检测 15 2例乳腺癌组织p2 1WAF1和LRP的表达水平。结果 在 15 2例乳腺癌组织中p2 1WAF1(+) 6 1例 (4 0 1% ) ,其中低表达 43例(70 5 % ) ,高表达 18例 (2 9 5 % ) ;LRP(+) 12 2例 (80 2 % ) ;p2 1WAF1的表达与乳腺癌组织腺管形成 (P <0 0 1)、细胞核多形性 (P <0 0 1)、核分裂象计数 (P <0 0 1)和组织学分级 (P <0 0 1)呈正相关。淋巴结转移组LRP的阳性率(88 6 % )明显高于无转移组 (73.2 % )。单因素预后分析显示 ,p2 1WAF1高表达组生存期低于低表达及无表达组 (P <0 0 1) ,p2 1WAF1(- ) /LRP(- )组生存期明显高于p2 1WAF1(+) /LRP(+)组 (P <0 0 1)。结论 ①p2 1WAF1不能提高LRP阳性乳腺癌患者的生存期 ,可能与其调控方向有关 ;
Objective To investigate the expression of p21 WAF1 and LRP and their prognostic significance in breast carcinomas. Method p21 WAF1 and LRP expression at protein level was determined by SP immunohistochemical technique in 152 patients with breast carcinomas. Results Positive immunostaining of p21 WAF1 was detected in 61 cases of primary breast carcinomas (40.1%), in which 43 cases had lower expression (70.5%) and 18, high expression (29.5%). LRP was detected in 122 cases (80.2%). p21 WAF1 expression level was correlated with the tubule formation ( P <0.01), nuclear atypia ( P <0.01), nuclear mitosis ( P <0.01) and histological grades ( P <0.01) in breast cancer tissues. The positive ratio of LRP expression was significantly higher in the axillary lymph nodes with metastasis than in the axillary nodes without metastasis (χ 2 =5.654, P <0.05). Univariate analysis demonstrated that disease free survival time was shorter in the patients with higher p21 WAF1 expression than in those with lower expression (χ 2 =6.92, P <0.01). Disease free survival time in the patients with p21 WAF1 (-)/LRP(-) was longer than those with p21 WAF1 (+)/LRP(+)(χ 2=7.512, P <0.01). Conclusions Expression of p21 WAF1 protein does not increase survival term in the patients with LRP(+) breast carcinoma. LRP(+) breast cancer cells are more apt to metastasize into axillary lymph nodes.
