摘要
目的 探讨白内障囊外摘除人工晶状体植入术中、术后角膜后弹力层脱离发生的原因及治疗措施。方法 对 1个月内施行白内障囊外摘除后房型人工晶状体植入术的 170例患者 170眼进行观察 ,将术中或术后后弹力层脱离按脱离范围不同分为 5个等级记录。结果 2 6眼发生不同程度的角膜后弹力层脱离 ,其中需临床处理者为 10眼 ,占 5 .88%。经前房注入空气泡、粘弹剂或缝线缝合后复位 ,未发生严重并发症。结论 手术切口靠前及器械反复进出切口是造成角膜后弹力层脱离的重要原因。早期复位可防止并发症的发生。
Objective To evaluate the causes and treatment of Descemet's membrane detachment when extracapsular cataract extraction and IOL implantation were performed.Methods 170 cases (170 eyes) which underwent extracapsular cataract extraction and IOL implantation in 1 month were observed.Descemet's membrane detachment was classified in 5 degrees according to the range of detachment.Results Descemet's membrane detachment occurred in 26 eyes.Among them,10 eyes needed clinic treatment.The rate was 5.88%.Desemet's membrane was replaced by injecting air,viscoelastic substances in anterior chamber or by suturing.No serious complications were observed.Conclusion Nearer incision and much time of intruding the appliances in the eyes may be the main causes of Descemet's membrane detachment.Early reposition can avoid complication.
