摘要
目的 观察全麻联合硬膜外阻滞在上腹部手术时对循环和内分泌功能的影响。方法3 0例择期上腹部手术患者随机分成两组 :A组全麻联合硬膜外组 ;B组单纯全麻。连续监测血压、心率及SpO2 。分别于麻醉前、插管后 2分钟、进腹探查时、术中 2小时和拔管后抽取静脉血测定血浆肾上腺素、皮质醇、血糖和胰岛素值。结果 A组血压、心率均较B组稳定 (P <0 0 5 )。A组麻醉前后内分泌各测定值无明显变化 ,而B组则显著升高 (P <0 0 1)。结论 全麻联合硬膜外阻滞在上腹部手术时对循环和内分泌影响小 ,是一种可行的麻醉方法。
Objective To study the effects of combined general epidural anesthesia on the endocrine and circulatory function during upper abdominal surgery.Methods Thirty patients undergoing elective upper abdominal surgery were randomly devided into two groups.Combined general epidural anesthesia was used in group A,and general anesthesia alone in group B.BP,HR and SpO 2 were monitored,and blood samples were taken before anesthesia,at 2min after intubation,abdominal exploration,2h during operation and soon after extubation for the measurements of epinephrine,cortisol,blood glucose and insulin.Results BP and HR were more stable in group A than those in group B( P< 0 05).The endocrine criteria measured were not markedly changed in group A and significantly elevated in group B.Conclusion Combined general epidural anesthesia has a less effect on the endocrine and circulatory function and is a good choice of anesthesia for upper abdominal surgery.
