摘要
目的探讨输卵管结扎术后子宫内膜形态学变化及月经失调的发病机理。方法分别对 12只正常大鼠及 14只扎管后大鼠的动情前期和动情期血浆雌二醇 (estradiol,E2 )和子宫内膜雌激素受体 (endometrialestrogenreceptor ,EEr)应用放射免疫分析法 (radioimmunoassay ,RIA)和过氧化物酶 抗过氧化物酶法 (peroxidaseantiperoxidase,PAP)进行测定 ,观察了子宫内膜的形态学变化。 结果动情前期血浆E2 水平和EEr含量明显高于动情期 ;子宫内膜粘膜大乳头及空泡状粘液细胞在动情前期较多 ,动情期明显减少 ;扎管后动情前期血浆E2 水平基本不变 ,但EEr含量明显减少 ,粘膜大乳头较对照组动情前期明显增多 ,空泡状粘液细胞减少 ;扎管后动情期血浆E2 维持在一个较高的水平 ,EEr含量及粘膜大乳头均较对照组动情期增加。结论结扎术后输卵管卵巢系膜间血管网被破坏 ,影响子宫的血供及激素由卵巢到子宫的传递 ,并破坏卵巢的黄体化过程 ,雌、孕激素比例失调 ,EEr合成发生障碍 。
ObjectiveTo explore the morphological changes of endometrium and incidence mechanism of abnormal menstruation after tubal ligation.MethodsPlasma estradiol(E 2) and endometrial estrogen receptor(EEr) in proestrus and estrus were studied in 12 normal rats and 14 rats with tubal ligation by radioimmunoassay(RIA) and peroxidase antiperoxidase(PAP) respectively.The morphological changes of the endometrium were also observed.ResultsE 2 and EEr in proestrus were significantly higher than those in estrus.A lot of projections of surface epithelium,endometrial glands and vacuous mucous cells existed in proestrus,but in estrus,they decreased significantly.In proestrus after tubal ligation E 2 did not change,but EEr declined greatly.The projections were higher and vacuous mucous cells were lower than those obtained in the control subjects in proestrus.In estrus after tubal ligation,E 2 maintained at a higher level,EEr contents and the mucous projections showed higher than those of the control subjects in estrus.ConclusionDestruction of vascular network among oviduct and mesovarium incurred after tubal ligation,influencing blood supply and direct hormone diffusion from ovary to uterus. The synthesis of EEr is out of order,which results in functional disorder of endometriam. MeSH ligation,tubal;receptors,estrogen;estradiol;endometrium/pathol;rats
