摘要
目的 探讨血清CA12 5联合EmAb测定诊断子宫内膜异位症的价值。方法 对经病理证实的 147例子宫内膜异位症 (EM组 )及 30例非子宫内膜异位症 (对照组 )患者 ,术前采用放射免疫法测定血清CA12 5水平 ,用酶联免疫吸附法(ELISA)检测血清抗子宫内膜抗体 (EmAb) ,并对 75例EM患者术后 1周的血清CA12 5水平进行测定。结果 EM组Ⅰ～Ⅳ期血清CA12 5水平 (U·ml-1, x±s)分别为 45 0 8±2 4 87、6 3 48± 19 32、10 9 43± 6 0 2 4、12 6 5 2± 86 74,对照组为 2 1 42± 8 31,EM组各期CA12 5水平明显高于对照组(P <0 0 1)。将EM组各期CA12 5水平与AFS评分作相关分析 ,两者均呈直线正相关 (P <0 0 1)。EM组EmAb阳性率为 6 4 6 3% ,明显高于对照组的 6 6 7% (P <0 0 5 ) ,且Ⅲ～Ⅳ期阳性率高于Ⅰ～Ⅱ期 (P <0 0 0 5 ) ;EM组血清CA12 5的敏感性及特异性分别为 89 41%、92 5 3% ;EmAb的敏感性为 6 4 6 3% ,特异性为 73 5 1% ;以血清CA12 5和EmAb均阳性为标准 ,敏感性稍有下降为 70 2 5 % ,特异性增加 10 0 %。75例术后 1周的CA12 5水平低于术前 (P <0 0 1)。结论 血清CA12 5联合EmAb测定对EM有较高的辅助诊断价值 ,2 0 0 1 0 4 0 5收稿 ,2 0 0 1 0 5 16修回作者单位 :安徽医科大学第一附属医院1 妇产?
Objective To study the diagnostic value of serum CA 125 and endometrial antibody (EmAb) detection on endometriosis (EM). Methods By using radioimmunoassay and enzyme linked immunosorbent assay (ELISA), the preoperative serum CA 125 concentration and EmAb were measured in 147 patients with endometriosis (EM group) and 30 patients with nonendometriosis (control group) and serum CA 125 concertration in the postoperative one week was measured in 75 patients with endometriosis. Results In the Ⅰ～Ⅳ stages of EM group, the serum CA 125 concentration was obviously higher than that of the control group ( P< 0 01). There was significantly positive correlation between AFS mark and serum CA 125 concentration in every stage of endometriosis. The positive rate (64 63%) of EmAb in EM group was obviously higher than that (6 67%) in control group ( P< 0 05), and the positive rate in Ⅲ～Ⅳ stage was significantly higher than that in Ⅰ～Ⅱ stage ( P< 0 05). The sensitivity and peculiarity of serum CA 125 are respectively to 89 41% and 92 53%. The sensitivity of EmAb is 64 63% and its peculiarity is 73 51%, when both serum CA 125 and EmAb were abnormal, the sensitivity slightly fall to 70 25%, but the peculiarity up to 100%. After one week of the postoperative serum CA 125 concentration in 75 patients with EM was obviously lower than preoperative concentration( P< 0 01). Conclusion Serum CA 125 combined with EmAb assay is useful in diagnostic assessment of EM and helpful to increase the diagnostic accuracy and to observe curative effect.
