我们用单克隆抗体(McAb)和兔补体体外净化骨髓中残留白血病细胞后对5例普通型急性淋巴细胞性白血病(CALL)患者进行了自身骨髓移植(ABMT)。抽出的骨髓细胞经McAb55加兔补体两次处理后,CALL抗原(CALLA)阳性细胞清除率可达2~3个对数级,骨髓有核细胞及CFU-GM回收率分别为48.7%~96.9%和39.5%~95.7%。外周血白细胞于移植后11~16d开始回升,14~29d升至1×10~9/L以上。23~53d出洁净室。例3于移植后4个月白血病复发, 8.2个月死亡。另4例无病活存分别为19^+、18^+、12^+和6^+个月,远期疗效正在定期随访中。
The methodology of immunoseparation of tumor cells from normal stem cells is described. The use of monoclonal antibody (McAb 55) specific for the common acute lymphoblastic leukaemia antigen and rabbit complement has been successful in five patients suffering from CALL in remission. The bone marrow treated was preserved at 21-24℃ for 52-56 hours. The patients were treated with the preconditioning measures and with ransplantation of au tologous bone marrow treated with the McAb 55 and complement. The transplantation was well tolerated and full bone marrow reconstitution was observed in all the five patients. One patient relapsed with GALLA-positive lymphoblasts in her blood four months after the transplant. She died of respiratory and circulatory failure 8.2 months after the transplantation. Others are surviving and leukaemia-free at 19^+, 18^+, 12^+ and 6^+ months posttransplantaton respectively.
