糖尿病植物神经病变的昼夜心率变化和心率变异 被引量:2

摘要 目的 :研究糖尿病患者植物神经病变与心率改变的关系。方法 :应用动态心电图对正常组和糖尿病植物神经病变 (DAN)患者 ,进行心率变异的线性时域、2 4h时序和非线性散点图分析。结果 :单纯糖尿病组 SDNN、SDANN和 PNN50 低于正常组 (P<0 .0 5 ) ;糖尿病 + DAN组各项线性时域分析指标均低于正常组和单纯糖尿病组 (P<0 .0 1 )。2 4h时序分析 :正常组和单纯糖尿病组有昼夜变化特征 ,而糖尿病 + DAN组昼夜变化特征丧失。散点图分析 :正常组多呈彗星状 ,单纯糖尿病组多呈鱼雷状 ,糖尿病 + DAN组多呈短棒状。结论 :心率变异变化与临床 DAN有较好的相符性 ,可望成为诊断 DAN的客观指标。 Objective: To investigate the circadian rhythmicity and heart rate variability(HRV) in diabetic with autonomic neuropathy(DAN). Methods: The time domain, time series analysis and Poincare plot of HRV were studied from ambulatory electrocardiogram in 22 diabetic patients without DAN (DAN-), 18 DAN (DAN+) and 30 healthy controls (group C). Results: SDNN, SDANN and PNN 50 were markedly decreased in DAN- compared with group C( P < 0.05 ). In DAN+ all indicators in time domain were decreased markedly compared with group C and DNA-( P <0.01). group C and DAN- exhibited a significant circadian periodicity. DAN+ had no circadian rhythmicity at all. In nonlinear study, Group C showed a comet shaped Poincare plot patterns and a clear circadian variability. Most DAN- exhibit a torpedo shaped patterns and the majority DAN+ had a short stick shaped patterns. Conclusion: The HRV and circadian periodicity have a good relationship with diabetic autonomic neuropathy.
