摘要
目的 :总结体外循环心脏直视手术的体会 ,探讨基层医院开展心脏直视手术的经验。方法 :对我院 1996年 5月～ 2 0 0 0年 8月收治的 32例先天性及后天性心脏病患者在低温体外循环下进行手术 ,其中房缺修补 8例 ,室缺修补 18例 ,瓣膜替换 6例。 3例合并其它畸形 ,予以同期矫治。结果 :体外循环时间 43～ 15 0min ,主动脉阻断时间 14～ 97min ;术后 30d内死亡 2例 (6 2 5 % ) ,并发症发生率 12 5 %。 30例随访 3月～ 4年均恢复较好。结论 :基层医院开展心脏直视手术 ,选择病例应本着安全第一的原则 ;加强体外循环管理 ,重视心肌保护 ,强调术后处理 。
OBJECTIVE:To summarize the experience of 32 patients underwent open heart surgery with extracorporeal circulation. METHOD:32 operations for the heart disease were performed from June 1996 to Augest 2000. There were 17 males and 15 females.Their ages ranged from 1 to 56 years old (mean 20.8 years old)with body weights of 9.5 to 84 kg. All patients underwent open heart surgery under hypothermic anaesthesia with extracorporeal circulation,which 8 were ASD repair, 18 VSD repair and 6 valve replacement.Other cardiac malformations were corrected in 3 cases.RESULTS:Extracorporeal circulation time was 43 to 150 min and the total time of aortic block was 14 to 97 min.The postoperative mortality within 30 days was 6.25% and incidence of complications were 12.5%.30 patients recovered well during three months to four years follow up.CONCLUSION:Good extracorporeal circulation management, proper Myocardial protection,correct postoperative management and prevention of complication were crucial to the succese of the operation.
